Early invasive cancer in colorectal polyps

Abstract

Background and aims: Treatment for invasive adenocarcinoma in colorectal polyps (malignant polyps) is controversial. The aim of this study was to evaluate our institutional treatment strategy for malignant polyps. Materials and methods: We reviewed the specimen slides and clinical records of eight patients who had malignant colorectal polyps removed by endoscopic polypectomy between 1996-1997. Results: The patient's ages ranged from 40 to 68 years (mean 53.5 yrs). Five of eight patients with malign polyps had early invasive colorectal carcinoma. Only one colectomy was needed because of surgical margin involvement. During the five year follow-up, no recurrence was recorded. Conclusion: Treatment of early invasive colorectal cancer with polypectomy seems to be adequate when resection margins are normal.

Kolorektal malign poliplerde erken invaziv kanser

Abstract

Giriş ve Amaç: Kolorektal poliplerde gelişen erken invaziv kolorektal kanserlerin tedavisi tartışmalıdır. Bu çalışmanın amacı, kliniğimizde saptanan ve tedavi edilen erken kolorektal kanserli olguların tedavi stratejilerini ve uzun süreli sonuçlarını değerlendirmektir. Gereç ve Yöntem: Bir yıl içinde Mart 1996-1997 tarihleri arasında hastanemizde rektosigmoidoskopi ve kolonoskopi ile saptanan polipler retrospektif olarak değerlendirilmiştir. Bulgular: Hastaların yaşları 40-68 (ortalama 35.5 yıl) arasında değişmekteydi. Malign polip saptanan 8 olgunun 5'inde erken invaziv kolorektal kanser saptandı. Cerrahi sınırda kanser saptanan bir olguda total kolektomi gerekliydi. Beş yıllık izlemde rekürrens gözlenmedi. Sonuç: Erken invaziv kolorektal kanser saptanan hastaların tedavisinde cerrahi sınırlar temizse polipektomi yeterli gibi görünmektedi
