Renal Pelvisin Spontan Rüptürü: Obstrüktif Üreter Taşının Nadir Bir Komplikasyonu

Öz

Toplayıcı sistemin spontan rüptürü üreter taşı kaynaklı obstrüksiyonların nadir görülen bir komplikasyonudur. Oluşan minimal kaçak spontan rezorbe olabilirken absorbe edilebilenden daha fazla miktardaki idrarın perirenal veya periüreteral alana sızmasıyla ürinom oluşur. Biz de spontan renal pelvis rüptürü neticesinde minimal idrar kaçağı ve ürinom gelişen iki olguya yaklaşımımızı ve literatürdeki tedavi stratejilerini sunmayı amaçladık. Bu iki olguda semptomların kontrolü ve ekstravazasyonun giderilebilmesi için double J stent kateterizasyon uyguladık. İki olguda da postoperatif kontrollerde sıvı koleksiyonunun kaybolduğu ve hidronefrozun gerilediği görüldü. Üreter taşının büyüklüğüne ve lokalizasyonuna, ürinomun genişliğine, hastanın diğer ek patolojilerine ve semptomlarının şiddetine göre konservatif, girişimsel ya da cerrahi tedaviler uygulanabilir

Spontaneous Rupture of Renal Pelvis: A Rare Complication of Obstructive Ureteral Stone

Abstract

Spontaneous rupture of the collecting system is a rare complication of ureteral stone obstruction. While minimal leakage can be absorbed spontaneously, a urinoma can develop if more urine than can be absorbed leaks to the perirenal or periureteral area. We aimed to present our approach to two cases with minimal urinary leakage and urinoma as a result of spontaneous renal pelvis rupture and to present the treatment strategies in the literature. In these two cases, we performed double J stent catheterization to control symptoms and eliminate extravasation. In both cases, postoperative controls showed that the fluid collection disappeared and the hydronephrosis retracted. Conservative, interventional, or surgical treatments may be performed depending on the size and localization of the ureteral stones, the size of the urinoma or other additional pathologies of the patient, and severity of symptoms
