Pediatrik Yaş Grubunda Adenotonsillektominin Tükürük ve Serum IgA, M, G Düzeylerine Etkilerinin İncelenmesi

Öz

Amaç: Palatin tonsiller ve adenoid doku Waldeyer halkasının majör komponentleridir. Üst hava yolunun ilk savunma mekanizması olan bu iki organın çıkarılmasının immün sistem üzerine olumsuz etkileri olup olmadığı halen soru işaretleri taşımaktadır. Çalışmanın amacı adenotonsillektominin humoral immünite üzerine herhangi bir etkisinin olup olmadığının ortaya konulmasıdır.Gereç ve Yöntemler: Çalışma Akdeniz Üniversitesi Tıp Fakültesi KBB Hastalıkları Anabilim dalı ve Biyokimya Anabilim dalında yapıldı. Hasta grubuna kronik tonsillit, tonsiller hipertrofi ve adenoid hipertrofi nedeni ile adenotonsillektomi endikasyonu konmuş çocuklar dahil edildi. Kontrol grubu ise kliniğimize başvuran, tamamen sağlıklı olduğu belirlenen çocuklardan oluşturuldu. Bulgular: Hasta grubu 29 %60,04 erkek ve 13 %39,96 kız çocuğundan oluştu. Kontrol grubu ise 14 %51,85 kız çocuğu ve 13 %48,15 erkekten oluştu. Kontrol grubunun tükürük Ig M ve Ig G değerleri ile hasta grubu IgM ve Ig G değerleri arasında istatistiksel olarak anlamlı fark bulunmadı p>0,05 ancak hasta grubunun preoperatif tükürük Ig A değerleri kontrol grubu tükürük IgA değerlerinden anlamlı şekilde yüksek bulunmuştur. Sonuç: Adenotonsillektominin kronik enfeksiyonlara bağlı gelişen birçok komplikasyona karşı oldukça etkin bir tedavi yöntemi olduğu artık çok iyi bilinmektedir. Ancak halen bu operasyon sonrası hastalarda immün defektin ortaya çıkabileceği konusunda şüpheler mevcuttur. Çalışmamızda tükürük ve serum IgM, IgG ve IgA düzeylerinde çeşitli değişiklikler saptadık. Özellikle postoperatif dönemde immünglobulin düzeylerinde düşüş olması immün sistem üzerine olumsuz bir etki olduğunu düşündürse de, immünglobulin düzeylerinin sağlıklı kontrol grubu değerleri ile bir fark göstermemesi ameliyatın humoral immünite defektine yol açmadığını düşündürmektedir

Evaluation of the Effects of Adenotonsillectomy on the Saliva and Serum IgA, M, G Levels in Pediatric Age Group

Abstract

Objective: Palatin tonsils and adenoid tissue are major components of the Waldeyer ring. The validity of the notion that the removal of these two organs, which constitute the first defense mechanism of the upper airway, has negative effects on the immune system is still debated. The aim of this study was to determine whether adenotonsillectomy has any effect on humoral immunity.Material and Methods: This study was carried out at the Akdeniz University Faculty of Medicine's Departments of ENT Diseases and Biochemistry. Children with chronic tonsillitis, tonsillar hypertrophy and adenoid hypertrophy were included in the patient group. The control group consisted of children who presented to our clinic and were found to be completely healthy.Results: The patient group consisted of 29 60.04% boys and 13 39.96% girls. The control group consisted of 14 51.85% girls and 13 48.15% boys. There was no statistically significant difference between salivary Ig M and Ig G values of the control group and the patient group p>0.05 , but preoperative salivary Ig A values of the patient group were significantly higher than in the control group Conclusion: Adenotonsillectomy is highly effective against chronic infection-related complications. However, there are still doubts that an immune defect will develop in the patients after this operation. In our study, we found several changes in saliva and serum IgM, IgG and IgA levels. Although the decrease in immunoglobulin levels in the postoperative period suggests a negative effect on the immune system, the fact that immunoglobulin levels do not show any difference with the healthy control group suggest that the operation does not cause a humoral immunity defect.
