The Achievements In Solid Organ and Composite Tissue Transplantation: A Historical Perspective

Abstract

ABSTRACT The idea of replacing an affected non-functioning organ with the same organ from a living organism dates back to anticient times. However, scientific discoveries and developments that provide the current level and the standards took place in late nineteenth century and more profoundly in the second half of the twentieth century. Kidney, liver and corneal transplants are surgical procedures that are commonly performed with low morbidity and mortality. Heart, lung, pancreas and small intestine transplants are the second most common transplants. Uterus, composite extremity and facial transplants are transplants performed with newer and different indications. Although organ transplantation is the definitive solution in the treatment of some end stage chronic diseases, the inadequacy of organ supply makes it inevitable to develop different treatment modalities and to work in new research areas. TARİHİ PERSPEKTİFLE SOLID ORGAN VE KOMPOZIT DOKU NAKILLERINDEKI GELIŞMELER

Öz

ÖZ Hastalıklı ve fonksiyon görmeyen bir organ yerine başka canlıdan alınan aynı organın nakledilmesi fikri çok eski çağlara dayanır. Ancak günümüzde erişilen düzey ve standartları sağlayan bilimsel buluşlar ve gelişmeler 19. yüzyıl sonu ve özellikle 20. yüzyıl ikinci yarısında gerçekleşmiştir. Böbrek, karaciğer ve kornea nakilleri yaygın olarak düşük morbidite ve mortalite ile uygulanan cerrahi işlemlerdir. Kalp, akciğer, pankreas ve ince barsak nakilleri de özellikle son dekadda sıklıkla yapılan nakiller arasına girmiştir. Uterus, kompozit ekstremite ve fasial nakiller daha yeni ve farklı endikasyonlarla yapılan ve günümüzde daha nadir uygulanan işlemler arasında sayılabilir. Organ nakli bazı son evre kronik hastalıkların tedavisinde kesin çözüm olmasına karşın organ teminindeki yetersizlik değişik tedavi modalitelerinin geliştirilmesini ve yeni araştırma alanlarında çalışmayı kaçınılmaz kılmaktadır.
