Obstrüktif Uyku Apnesi Tanısında Epworth Uykululuk Ölçeğinin Ortak Değişkene Göre Düzeltilmiş İşlem Karakteristik Eğrisi ile Değerlendirilmesi

Abstract

Amaç: Obstrüktif uyku apnesi OSA tanısında Epworth uykululuk ölçeğinin ESS başarısının değerlendirmesinde ortak değişken etkisinin araştırılması ve etkili ortak değişkenler için düzeltme yapılarak analiz edilmesi amaçlanmıştır. Gereç ve Yöntemler: Çalışmaya Nisan 2016 - Ağustos 2016 tarihleri arasında Akdeniz Üniversitesi Kulak Burun Boğaz Hastalıkları Anabilim Dalı’ nda polisomnografi ile OSA tanısı konulan apne hipopne indeksi, AHİ >5 ve ESS anketi olan 340 hasta alınmıştır. ESS’ nin OSA tanısındaki başarısı AHİ ≥ 15 ve AHİ ≥ 30 olmak üzere iki farklı düzende incelenmiştir. Analizde klasik işlem karakteristik eğrisi İKE ve ortak değişken etkisi düzeltilmiş İKE analizi kullanılmıştır. Bulgular: Ortak değişken etki tahminleri tüm olası ortak değişkenlerin modele katıldığı İKE modeli ile incelendiğinde, diğer değişkenler de modeldeyken sadece boyun çevresinin anlamlı etkisi olduğu gözlenmiştir P

Covariate Adjusted Receiver Operating Characteristic Curve Analysis for the Evaluation of the Epworth Sleepiness Scale in Diagnosis of Obstructive Sleep Apnea

Abstract

Objective: The purpose of this study was to investigate the effect of covariates in the diagnostic performance of the Epworth Sleepiness Questionnaire ESS in obstructive sleep apnea OSA and to analyze it by covariate adjusted analysis.Material and Methods: This retrospective cohort study enrolled 340 consecutive patients, who completed the ESS questionnaire, underwent complete polysomnographic evaluation and were diagnosed with OSA apnea hypopnea index, AHI >5 at our accredited sleep disorders center between April 2016 and August 2016. Diagnostic accuracy of ESS was evaluated for two different settings; AHI ≥ 15 and AHI ≥ 30. In the statistical analysis, classical receiver operating characteristic ROC curve and covariate adjusted ROC curve analyzes were used.results: In the ROC regression model, where all covariates were included, only the covariate effect of neck circumference was found statistically significant P
