TÜRKİYE’DE SAĞLIK KURUMLARINTN PERFORMANS ANALİZİ: BİR VZA UYGULAMASI

Abstract

Türkiye’de sağlık politikaları geliştirmek, uygulamak ve sağlık hizmetleri sunmak resmi olarak Sağlık Bakanlığı Tun görevidir. Sağlık hizmetleri kaynaklarının ülke genelinde dengeli dağılımı, etkili ve verimli kullanımı Sağlık Bakanlığı Tun sorumluluğundadır. Sağlık harcamalarının yükselmesi, kalitesinin artması ve sektörün rekabetçi bir yapıda daha etkin bir hizmet sunabilmesi, sektörün önemli bir kısmını oluşturan devlet hastanelerinin kaynaklarını daha etkin kullanmasını gerektirmektedir.
Bu çalışmada Türkiye’de Sağlık Bakanlığına bağlı hizmet veren 81 ilde 846 adet Devlet Hastanesinin (yataklı tedavi kurulularının) iller ve bölgeler bazında 2003, 2004, 2005, 2006 yılları itibariyle performanslarının değerlendirmesi yapılmıştır. Performans değerlendirmesi yapılırken metodolojik olarak Veri Zarflama Analizi (VZA) kullanılacaktır. VZATun sunduğu imkanlar çerçevesinde ayrı ayrı modeller (CCR ve BCC) kullanılarak ölçeğe göre sabit ve değişken getiri durumlarına göre hastaneler, iller ve bölgeler bazında değerlendirilmiştir. Performans değerlendirmesinde; hastanelere ait uzman ve pratisyen hekim sayıları, yatak sayısı ve döner sermaye harcamaları gibi değişkenler girdi olarak kullanılacak, poliklinikte ve yatarak tedavi gören hasta sayısı, ölen hasta sayısı, yapılan ameliyat sayıları, döner sermaye gelirleri ve doğum sayısı gibi değişkenler de çıktı olarak kullanılmıştır. Analiz sonrasında etkin olan ve olmayan hastaneler tespit edilecek, olmayanların etkinliği için potansiyel iyileştirme önerileri geliştirilmiştir.


ABSTRACT
The Ministry of Health is responsible to develop health, execute health policy and to deliver health services in Turkey. The Ministry is
accountable to allocate and use health resources effectively. Growing health expenditures, increased quality and competition in the health sector put pressure on government hospitals to use their resources more efficiently if they want to compete with the private hospitals.
This study wills evaluate the performance of 846 state hospitals (the hospital with patients beds) from 81 provinces in 2003, 2004, 2004, 2005 and 2006. Performance evaluation will be made by using Data Envelopment Analysis Opportunities given by DEA, different models (CCR and BCC) will be used to evaluate the hospital in terms of fixed yield and variable yield. The hospitals will be grouped in terms of provinces and regions. Performance evaluation will use the number of medical practitioner and medical experts, the number of patient beds and the expenditure of circulating capital as inputs and the number of inpatient, dead patients, surgery operations, yields of circulating capital and new bom as output. After analysis effective and ineffective hospitals will be determined and recommendations will be made to improve for ineffective organisation.
