İnmeli Hastalarda Disfaji Yönetimine Yönelik Kanıta Dayalı Bakım Rehberi Geliştirilmesi

Abstract

Amaç: Bu çalışmanın amacı, inme sonrası disfaji yönetiminde hemşirelere yönelik kanıta dayalı bakım rehberi geliştirmektir.
Yöntem: İnme sonrası disfaji yönetiminde hemşirelere yönelik kanıta dayalı De Novo rehber, 1 Ocak -30 Aralık 2017 tarihleri arasında kanıta dayalı rehber süreç geliştirme yaklaşımı kullanılarak geliştirildi.
Bulgular: Literatür incelenerek 2000-2017 tarihleri arasında yayınlanan çalışmalar derlendikten sonra, bunların kanıtla ilişkileri yönünden araştırmalar sınıflandırıldı. Kanıt kalitesinin değerlendirilmesi ve analizi yapılarak kanıt tabloları oluşturuldu. Rehberde yer alan kanıta dayalı öneriler 17 çalışmanın ve 10 rehberin gözden geçirilmesinden elde edildi. Elde edilen kanıtlar belirlenen altı alana (Tarama, Değerlendirme, İzlem, Tedavi, Konsültasyon ve Eğitim) ayrıldı.
Sonuç: İnme sonrası gelişen disfajinin erken dönemde saptanması ve komplikasyonlarının azaltılmasında, geliştirdiğimiz rehberin hasta bakım kalitesini artırmaya ve komplikasyonların önlenmesine önemli katkı sağlanması beklenmektedir.

Development of Evidence-Based Care Guidelines for the Management of Dysphagia in Patients with Stroke

Abstract

Objective: The evidence-based De Novo guideline for nurses in the management of post-stroke dysphagia was developed between January 1 and December 30, 2017 using the evidence-based guideline process development approach.
Methods: The evidence-based De Novo guideline for nurses in the management of post-stroke dysphagia was developed between January 1 and December 30, 2017 using the evidence-based guideline process development approach.
Results: After reviewing the literature and compiling the studies published between 2000-2017, the studies were classified in terms of their relation to the evidence. Evidence tables were created by evaluating and analyzing the quality of evidence. Evidence-based recommendations in the guideline were derived from a review of 17 studies and 10 guidelines. The evidence obtained was divided into six identified areas (Screening, Evaluation, Follow-up, Treatment, Consultation and Education).
Conclusion: It is expected that the guide we have developed will make a significant contribution to the early detection of post-stroke dysphagia and the reduction of its complications, to improve the quality of patient care and to prevent complications.
