Malignant Melanoma Metastasizing to the Stomach

Abstract

Introduction:
Malignant melanoma (MM) can metastasise to nearly all organs of the human body.
Involvement of stomach is very rare.

Case report:
52-year-old female patient applied to our hospital with abdominal pain and
fatigue. Endoscopy revealed polypoid lesion in stomach.  Histopathologically, there were atypical
cells in lamina propria and brown pigment in some cells. These cells were
positive with Melan-A and HMB-45 and negative with Cytokeratin 7 and
Cytokeratin 20, immunohistochemically. This case was reported as “consistent
with MM”. We have reached to former pathology report of the enucleation
operation, with diagnosis of “MM with epithelioid features”. Tumor was
localised in the right bulbus oculi with pathological stage pT1a. Systemic scan
also revealed metastatic masses in liver.

Conclusion:
MM can simulate many other lesions clinically, endoscopically and
histopatholgically and must be keeped in mind all the time, especially in cases
with unknown primary. The patients should be screened for the primary tumor.

Mide Metastazı Yapan Malign Melanom

Abstract

Giriş:
Malign melanom (MM) insan vücudunda tüm organlara metastaz yapabilmektedir.
Gastrointestinal sistem metastazı sık olmakla beraber gastrik mukoza tutulumu
oldukça nadirdir.

Olgu sunumu:
52 yaşında kadın hasta halsizlik ve karın ağrısı şikayeti ile hastanemize
başvurdu. Yapılan mide endoskopisinde polipoid lezyon izlendi. Mide
biyopsisinde lamina propriada atipik hücreler izlendi. Hücrelerin bir kısmında
intrasitoplazmik kahverenkli pigment birikimi dikkati çekti.
İmmünohistokimyasal olarak tanımlanan lezyonu oluşturan hücreler Sitokeratin 7
ve Sitokeratin 20 ile negatif, Melan-A ve HMB45 ile pozitif boyandı. Olgu “MM
ile uyumlu” olarak raporlandı. Hastanın iki yıl önce dış merkezde yapılmış olan
enukleasyon materyaline ait patoloji raporuna ulaşıldı ve olgunun sağ bulbus
okuli posterior yerleşimli, evre pT1a olarak değerlendirilmiş, “Epitelioid
Morfolojide MM” tanısı aldığı görüldü. Ayrıca sistemik taramalarda karaciğerde
metastatik lezyonlar saptandı.

Sonuç:
Klinik, endoskopik ve histomorfolojik olarak birçok patolojiyi taklit edebilen
MM; özellikle primeri bilinmeyen durumlarda daima akılda tutulmalı ve hastalar
primer tümör açısından klinik olarak sorgulanmalıdır.
