Gonokokkal enfeksiyonlar ve tedavisi

Abstract

ÖZ



Gonore Neisseria gonorrhoeae’nin neden olduğu ikinci en sık rapor edilen
cinsel yolla bulaşan enfeksiyondur. 25 yaşın altındakiler ve yüksek riskli
cinsel davranışları olanlar risk altındadır. Esas olarak enfeksiyon üretra,
serviks, rektum, farenks ve konjunktivanın mukoz membranlarını tutar.
Erkeklerde üretrite, kadınlarda servisite neden olur. Erkekler genellikle
semptomatiktir. Dizüri ve penil akıntı en sıklıkla görülür. Kadınların %95’i
semptomsuzdur. Semptomatik olgularda ise klinik genelikle hafiftir ve akut
sistiti veya vaginiti taklit eder. Gram boyama, kültür ve nükleik asit
amplifikasyon testleri ile tanı konulabilir. Hastalar genellikle Chlamidya trachomatis ile birlikte
enfektedir. Bu nedenle kombine tedavi önerilmektedir. Komplike olmayan
servikal, üretral, anorektal ve farengeal infeksiyonların tedavisinde
seftriaksonla (250 mg intramuskuler tek doz) birlikte ya azitromisin (1 g oral
tek doz), veya doksisiklin (100 mg günde iki kez yedi gün) etkindir.
