Antimicrobial Resistance in Clinical Strains of Stenotrophomonas maltophilia: 10-year surveillance

Abstract

Stenotrophomonas maltophilia is an opportunistic pathogen of increasing importance particularly in the immunocompromised patient population and community-acquired cases. Infections caused by S. maltophilia are difficult to manage because of its intrinsic resistance to many antibiotics. The aim of this study is to investigate the changes in the resistance rates of S. maltophilia strains over ten years and to evaluate the risk factors for S. maltophilia infection. In this study; antimicrobial resistance rates of 276 S. maltophilia strains isolated from various clinical specimens between January 2010-September 2019 were investigated retrospectively. Identification and antimicrobial susceptibilities of the isolates were studied by VITEK2 Compact (BioMérieux, France). A total of 276 clinical isolates of S. maltophilia were included in the study, and only one isolate was taken from each patient. Of the isolates, 20.7% were isolated from outpatient and 79.3% from hospitalized patients. Of the hospitalized patients, 38.8% (85/219) were admitted to the intensive care unit. It was detected that 64.5% of the patients were male, 35.5% were female; the mean age was 60 for men and 66 for women. 53.6% (n=148/276) of the isolates included in the study were isolated from lower respiratory tract samples. Antibiotic susceptibilities to trimethoprim-sulfamethoxazole (SXT), levofloxacin, and ceftazidime were; 92.8%, 93.1%, and 60.1%, respectively. When the antibiotic resistance rates between 2010-2014 and 2015-2019 were compared, it was found that the rate of resistance to ceftazidime (p<0.001) and levofloxacin (p<0.001) decreased in the second period, but the rate of SXT resistance increased (p=0.485). In conclusion, levofloxacin and SXT continue to be most effective against S. maltophilia isolates in our study. However, although not statistically significant, caution should be exercised in terms of increasing rates of resistance to SXT. In addition, since antimicrobial resistance rates may vary, each hospital should follow its own antimicrobial resistance rates and determine its empirical treatment policy according to its resistance status.

STENOTROPHOMONAS MALTOPHILIA KLİNİK SUŞLARINDA ANTİMİKROBİYAL DİRENÇ: 10 YILLIK SÜRVEYANS

Abstract

Stenotrophomonas maltophilia, özellikle bağışıklığı baskılanmış hasta popülasyonunda ve toplum kökenli vakalarda önemi giderek artan fırsatçı bir patojendir. S. maltophilia'nın neden olduğu enfeksiyonların tedavisi birçok antibiyotiğe karşı intrensek direnci nedeniyle oldukça zordur. Bu çalışmanın amacı, on yıllık bir süre içinde izole edilen S. maltophilia suşlarının yıllar içerisindeki direnç oranlarının değişiminin irdelenmesi ve S. maltophilia enfeksiyonu için risk faktörlerinin değerlendirilmesidir. On yıllık süre boyunca (Ocak 2010 – Aralık 2019) çeşitli klinik örneklerden izole edilen S. maltophilia klinik izolatlarının antibiyotiklere direnç oranları retrospektif olarak incelenmiştir. İzolatların identifikasyonu ve antimikrobiyal duyarlılıkları VITEK2 Compact (BioMérieux, Fransa) ile çalışılmıştır. Çalışmaya toplam 276 S. maltophilia izolatı dahil edilmiş, her hastadan tek izolat alınmıştır. İzolatların %20.7'si ayaktan, %79.3'ü hastanede yatan hastalardan izole edilmiştir. Yatan hastaların %38.8’inin (85/219) yoğun bakım ünitesindeki hastalardan oluştuğu gözlenmiştir. Hastaların %64.5’inin erkek, %35.5’inin kadın; yaş ortalamalarının erkeklerde 60, kadınlarda 66 olduğu saptanmıştır. Çalışmaya dahil edilen izolatların %53.6’sı (148/276) alt solunum yolu örneklerinden izole edilmiştir. Antibiyotik duyarlılıkları trimetoprim-sülfametoksazol (SXT), levofloksasin ve seftazidim için sırasıyla %92.8, %93.1 ve %60.1 olarak tespit edilmiştir. 2010-2014 yılları arasındaki ve 2015-2019 yılları arasındaki antibiyotik direnç oranları karşılaştırıldığında ikinci dönemde seftazidim (p<0.001) ve levofloksasin (p<0.001) direnç oranının düştüğü ancak SXT direnç oranının arttığı tespit edilmiştir (p=0.485). Sonuç olarak çalışmamızdaki S. maltophilia izolatlarına karşı levofloksasin ve SXT etkisini sürdürmektedir. İstatistiksel olarak anlamlı olmamasına ragmen, SXT’ye karşı artan direnç oranları açısından dikkatli olunmalıdır. Ayrıca antimikrobiyal direnç oranları değişiklik gösterebileceği için her hastane kendi antimikrobiyal direnç oranlarını takip etmeli, ampirik tedavi politikasını kendi direnç durumuna göre belirlemelidir.
