Çeşitli Klinik Örneklerden Elde Edilen Stenotrophomonas maltophilia İzolatlarının Dağılımı ve Antibiyotik Duyarlılık Profili

Abstract

Stenotrophomonas maltophilia, yenidoğan ve ileri yaş grubundaki kişilerde, solunum yolu enfeksiyonları başta olmak üzere nozokomiyal ya da toplum kaynaklı çeşitli enfeksiyonlar ile ilişkilendirilmiş, kısıtlı tedavi seçeneği olan fırsatçı bir patojendir. Bakterinin antimikrobiyal direnci yıllar içinde artmakta ve coğrafi bölgeler arasında farklılıklar göstermektedir. Bu çalışma ile hastanemizde üç yıl boyunca izole edilen S. maltophilia suşlarının dağılımı ve antibiyotik duyarlılığının retrospektif olarak araştırılması amaçlanmıştır. Ocak 2022- Aralık 2024 tarihleri arasında çeşitli klinik örneklerden izole edilen S. maltophilia suşları incelenmiştir. Suşların identifikasyonu ve antimikrobiyal duyarlılığının belirlenmesi için Phoenix otomatize sistemi (Becton Dickinson Diagnostic Instrument Systems, Sparks, ABD) kullanılmıştır. Çalışma süresince toplam 233 S. maltophilia izolatı elde edilmiştir. Suşlar en sık solunum yolu örneklerinden (%66.5) izole edilmiştir. Suşların %60.3’ü çocuk ve yoğun bakımlarda tedavi gören hastalardan, %11.1’i ise poliklinikten başvuran hastalardan izole edilmiştir. Hastaların 157’si (%67.4) 65-95 yaş aralığında bulunmuştur. Suşların %90.12’si (210/233) trimetoprim/sülfametoksazole (TMP/SXT), %80.34’ü (184/229) ise levofloksasine duyarlı bulunmuştur. Sonuç olarak, verilerimiz yoğun bakımda yatan ileri yaş grubundaki kişilerin S. maltophilia enfeksiyonu gelişimi açısından dikkatle takip edilmesi gerektiğini düşündürmektedir. Hastanemizde izole edilen S. maltophilia suşları için en etkili antimikrobiyal TMP-SXT iken, yıllar içinde levofloksasin duyarlılığındaki azalma dikkat çekicidir. S. maltophilia enfeksiyonlarında önemli tedavi seçenekleri olan bu iki antimikrobiyal için duyarlılık durumu yeni çalışmalarla takip edilmelidir.

Distribution and antibiotic susceptibility profile of Stenotrophomonas maltophilia isolates obtained from various clinical specimens

Abstract

Stenotrophomonas maltophilia is an opportunistic pathogen with limited therapeutic options that has been associated with various nosocomial or community-acquired infections, especially respiratory tract infections in newborns and elderly people. Antimicrobial resistance of the bacterium increases over the years and varies between geographical regions. The aim of this study was to retrospectively investigate the distribution and antibiotic susceptibility of S. maltophilia strains isolated in our hospital for three years. S. maltophilia strains isolated from various clinical samples between January 2022 and December 2024 were analyzed. Phoenix automated system (Becton Dickinson Diagnostic Instrument Systems, Sparks, USA) was used for bacterial identification and antimicrobial susceptibility testing. A total of 233 isolates were obtained during the study period. In the study, strains were most frequently isolated from respiratory tract samples (66.5%). 60.3% of the strains were isolated from patients treated in pediatric and adult intensive care units, while 11.1% were isolated from outpatients. 157 (67.4%) of the patients were between 65-95 years of age. Of the strains, 90.12% (210/233) were susceptible to trimethoprim/sulfamethoxazole (TMP/SXT) and 80.34% (184/229) to levofloxacin. In conclusion, our data suggest that elderly patients hospitalized in the intensive care unit should be carefully monitored for the development of S. maltophilia infection. While TMP-SXT was the most effective antimicrobial for S. maltophilia strains isolated in our hospital, the decrease in levofloxacin susceptibility over the years is remarkable. The susceptibility status for these two antimicrobials, which are important treatment options in S. maltophilia infections, should be followed up with new studies.
