Diagnostic approaches to soft tissure disorders of the shoulder joint (A comparison of the clinical, arthrographic, ultrasonographic, computerized tomographic and arthroscopic findings)

Abstract

Incrasing research about sogft tissue disorders of the shoulder joint engaged latest diagnostic technics on this subject. Our purpose is to define how these technice can be engaged in diagnosis of these disorders. We retrospectively analyzed 63 cases who admitted between 1988-1989 because of shoulder disorders excluding fractures and dislocations of be the shoulder. These cases. following systematic and physical examination were evaluated using radiologic, arthrographic, ultrasonographic. computerised tomographic and arthroscopic technics for definitive diagnosis. In 13 cases cuff with rotator disease endorsement of clinical findings by arthrographic and ultrasonographic findings helped us reaching the diagnosis. In diagnosis of impingment syndrome dynamic utilization of ultrasonography can be useful. Clasification of primary disorders of the shoulder joint following shoulder dislocation or insatbility besides secondary disorders developing due to these disorders outlines the strategy of the surgical procedure. Because of this, these diagnostic technics were utilized until we reached the diagnosis in our cases. Among them arthroscopy and camputerised tomography were the most rewarding. For other disorders diagnostic technics stated above were as clinical findings necessitated.

Omuz ekleminin yumuşak doku patolojilerine tanısal yaklaşımlar (Klinik, artrografik, ultrasonografik, bilgisayarlı aksiyel tomografik ve artroskopik bulguların karşılaştırılması)

Abstract

Omuz ekleminin yumuşak doku patolojileri üzerine giderek artan çalışmalar günümüz gelişmiş tanı yöntemlerini konu üzerine yoğunlaştırmıştır. Amaç bu patolojiler ve düzeylerini ortaya koyabilmek için günümüz görüntüleme yöntemlerinden ne şekilde yararlanılacağının bilınmesidir. Bu amaç doğrultusundaki çalışmamızda 1988-1989 acil kırık ve çıkık dışındaki çeşitli omuz patolojileri tesbit edilen 63 olguda kesin tanıya götüren yöntemleri retrospektif olarak inceledik. Bu olgularda sistematize değerIendirme ve fizik muayene sonrası bulgularına göre; radyolojik , artrografik, ultrasonografik, bilgisayarlı aksiyel tomografik ve artroskopik yöntemlerle tanıya ulaşıldı. Rotator Cuff lezyonu olan 13 olgudaki klinik bulguların artrografik ve ultrasonografik bulgularla desteklenmesi kesin tanıya götürücü olmuştur. Impingment veya sıkışma sendromunda ise, ultrasonografinin dinamik amaçlı kullanımı yararlı olmaktadır. Habitüel omuz çıkığı ve omuz instabilitesindeki primer ve buna bağlı olarak sekonder patolojilerin ortaya konulması cerrahi yöntemin seçimini belirleyici kriter olmaktadır. Bu nedenle çalışmamızdaki olgularda patolojik bulgulara ulaşana kadar tanı yöntemleri kombine edilmiştir. Bunlar içerisinde bilgisayarlı aksiyel tomografi ve artroskopi en değerlisi olmuştur. Diğer patolojilerde ise kliniğin yön verdiği şekilde tanı yöntemlerinden faydalanılmıştır.
