The results of radical reduction in congenitally dislocated thirty hips in and over six years old

Abstract

30 hips of 25 patients in and over six years old on which radical reduction operation had been performed, were examined after a mean of three years postoperatively. Avascular necrosis of femoral head was established in 24 hips (80%), Ieg Ienght discrepancy in 14 patients (56%), hip stiffness in 11 hips (37 %), subluxation and redislocation in 1O hips (33%). Hips were evaluated according to Trevor's criterions and succesful result was obtained in only 11 hips (37%). Therefore, it was thought radical reduction has Iittle chance of success in congenitally dislocated hips in and over six years of age. Two boys who were 11 years old and had unilateral dislocation, had succesful results. We had six patients who had bilaterally dislocated hips. Only one of them was better than preoperative functional condition. The rate of success of radical reduction and over six years of age increases ind the hands of an orthopedic surgeon experienced in congenital dislocation of the hip. Succesful results can be obtained even in eleven years of age. Unfortunately, bilateral dislocated hips have Iess chance even in the hands of an experienced surgeon. If the patient having bilaterally dislocated hips is in or over six years old, radical reduction should be avoided.
