Treatment of unstable fractures of the spine with the Cotrel-Dubousset instrumentation
Abstract
This study reviews 13 unstable spinal fractures, which were stabilized with the Cotrel-Oubousset instrumentation, at the Ankara University Ibn-i Sina Hospital between Nowember 1989 and July 1991. AII fractures were considered ''unstable'' according to Denis “three column spine concept” and 10 of them involved middle and posterior columns, 3 of them anterior, middle and posterior columns. The age at operation ranged from 19 to 64 years with an average of 38.6 years. The average follow-up period was 7.1 months (2-18- months). All patients were evaluated both radiographically and clinically. The average local kyphosis angle was 30.7° preoperatively, and 20.7° postoperatively (32% improvement). The average correction Ioss was 2.5°. The average anterior vertebral body height improved from 49 % to 58 % and the average posterior vertebral height from 79% to 86%. A postoperative improvement of one step in the "Frankel classification" occured in 2 patients. Complications included 1 exitus, due to ARDS and 1 deep infection, treated with surgical drainage and appropriate antibiotic therapy. As Cotrel-Oubousset instrumentation achieves stable osteosynthesis in unstable fractures of the spine, and shortens the time of ambulation and hospitalization, it is an important alternative in this section
