Arthroscopic treatment of osteochondral lesions of the talus
Abstract
Four cases (two females, two males) with osteochondral lesions of talus were treated arthroscopically and followed for an average of 9.5 months (range 8-11 months) prospectively. Mean age was 36 years (range 21-43 years). Mean duration of symptoms from the onset to the operation was 8.5 years (range 2-23 years). All but one patient reported previous trauma, two patients were recreational athletes. Lesions were located anterolaterally in two patients and anteromedially in the other two. Plain radiologic evaluation demonstrated stage II lesions in 2 patients, stage I lesion in one and stage IV in another. In all four patients the talar bed was debrided and drilling was performed. Preoperative AOFAS scores increased from an average of 72.8 to 84.3 postoperatively (p<0.025). Qualitative results were excellent in two patients, good in one and moderate in another. A minor complication was observed in one patient, that was, hypoesthesia due to injury of a branch of the superficial peroneal nerve. Arthroscopy should be the method of choice in osteochondral lesions of the talus, since arthroscopy is a minimal invasive technique compared to arthrotomy and since both techniques give similar results.
