Arthroscopic treatment of osteochondral lesions of the talus

Abstract

Four cases (two females, two males) with osteochondral lesions of talus were treated arthroscopically and followed for an average of 9.5 months (range 8-11 months) prospectively. Mean age was 36 years (range 21-43 years). Mean duration of symptoms from the onset to the operation was 8.5 years (range 2-23 years). All but one patient reported previous trauma, two patients were recreational athletes. Lesions were located anterolaterally in two patients and anteromedially in the other two. Plain radiologic evaluation demonstrated stage II lesions in 2 patients, stage I lesion in one and stage IV in another. In all four patients the talar bed was debrided and drilling was performed. Preoperative AOFAS scores increased from an average of 72.8 to 84.3 postoperatively (p<0.025). Qualitative results were excellent in two patients, good in one and moderate in another. A minor complication was observed in one patient, that was, hypoesthesia due to injury of a branch of the superficial peroneal nerve. Arthroscopy should be the method of choice in osteochondral lesions of the talus, since arthroscopy is a minimal invasive technique compared to arthrotomy and since both techniques give similar results.

Talus osteokondral lezyonlarında artroskopik yaklaşım

Öz

Talus osfeokondral lezyonu tanışı ile artroskopik tedavi uyaulanan 4 olgu prospektif yöntemle ortalama 9.5 ay takip edildi. İkisi kadın, ikisi erkek olan hastaların yaş ovalamaları 36 yıldır (21-43 yıl arasında). Yakınmaların başlangıcı ile ameliyat arasında geçen süre ortalama 8.5 yil (2-23 yıl) olarak bulunmuştur. Bir hasta hariç tüm hastalarda travma öyküsü bulunmakta, hastaların ikisi rekreasyonel amaçlı spor yapmaktadır. 2 hastada lezyon anterolateralde. 2 hastada ise anteromedialdedir. Direkt radyolojik evrelendirme ile 2 hasta evre II, birer hasta da evre l ve evre IV olarak sınıflandırıldı. 4 hastada da lezyonun yatağı artroskopik yöntemle canlandırıldı, ve drilleme uygulandı. Ameliyat öncesi AOFAS skorlarınm ortalaması 72.8 iken ameliyat sonrasında 84.3'e yükseldi (p<0.025). Katitatif sonuçlara göre de 2 mükemmel, l iyi ve 1 ona sonuç elde edildi. Bir hastada yüzeyel peroneal sinirin daimin lezyonuna bağlı hipoestezi komplikasyonu gözlendi. Artroskopik tekniğin görece az invazif bir yöntem olması ve talus lezyonlarında artroskopik yöntemle elde edilen sonuçların artrotomi ile elde edilenlerden farklı olmaması nedeniyle artroskopik yöntemin tercih edilecek yöntem olduğu sonucuna varıldı.
