The results of surgical treatment in Kienbock’s disease

Abstract

Kienböck’s disease still remains a problem. If it has not been treated, may predispose to wrist instability, carpal collaps and degenerative arthritis. So Kienböck’s disease is a disabling disease. Usually carpal collaps and structural derangement progressively increase. The aim of this study was to evaluate the problems in the treatment of Kienböck’s disease especially stage III. Twenty-two patients with Kienböck’s disease were treated by various surgical procedures at our Hand and Microsurgery Hospital between 1987-1997. Stages of Kienböck’s disease were evaluated on x-ray examination according to the classification of Lichtman et al. Age at the time of operation averaged 26 years old. (min.15-max. 47) Patients were classified as follows; 11 stage IIIb, 8 stage IIIa, 2 stage II, 1 stage I We report the average 35 months follow up duration of 22 wrists after radial shortening osteotomy (11 patients), STT fusion (6 patients), CH fusion (3 patients), proximal row carpectomy (1 patient), drilling (1 patient), assessing symptoms and radiographic parameters such as Stahl index, ulnar variance, carpal height ratio. Objective assessment included measurement of wrist range of motion , grip strength patients, in the group of intercarpal fusion had significantly lower degrees ROM (37.5 %) and grip strength (55%). However in radial osteotomy group wrist mobility and patient satisfaction were higher. According to Lichtman scoring system, our results were as follows; 5 patients excellent ( 25%), 11 patients good (%55), 3 patients fair (%15), 1 patient poor (%5). Although our radial shortening results were satisfactory as reported in other literature, intercarpal fusion (STT) give rise to lower functional results. Since fusion operation is an intraarticular technically demanding procedure, there is no doubt that it requires more experience. It should be remembered that, in stage IIIb, even extraarticular procedures may also result in functionally good and painless wrist.
