The results of surgical treatment in Kienbock’s disease

Abstract

Kienböck’s disease still remains a problem. If it has not been treated, may predispose to wrist instability, carpal collaps and degenerative arthritis. So Kienböck’s disease is a disabling disease. Usually carpal collaps and structural derangement progressively increase. The aim of this study was to evaluate the problems in the treatment of Kienböck’s disease especially stage III. Twenty-two patients with Kienböck’s disease were treated by various surgical procedures at our Hand and Microsurgery Hospital between 1987-1997. Stages of Kienböck’s disease were evaluated on x-ray examination according to the classification of Lichtman et al. Age at the time of operation averaged 26 years old. (min.15-max. 47) Patients were classified as follows; 11 stage IIIb, 8 stage IIIa, 2 stage II, 1 stage I We report the average 35 months follow up duration of 22 wrists after radial shortening osteotomy (11 patients), STT fusion (6 patients), CH fusion (3 patients), proximal row carpectomy (1 patient), drilling (1 patient), assessing symptoms and radiographic parameters such as Stahl index, ulnar variance, carpal height ratio. Objective assessment included measurement of wrist range of motion , grip strength patients, in the group of intercarpal fusion had significantly lower degrees ROM (37.5 %) and grip strength (55%). However in radial osteotomy group wrist mobility and patient satisfaction were higher. According to Lichtman scoring system, our results were as follows; 5 patients excellent ( 25%), 11 patients good (%55), 3 patients fair (%15), 1 patient poor (%5). Although our radial shortening results were satisfactory as reported in other literature, intercarpal fusion (STT) give rise to lower functional results. Since fusion operation is an intraarticular technically demanding procedure, there is no doubt that it requires more experience. It should be remembered that, in stage IIIb, even extraarticular procedures may also result in functionally good and painless wrist.

Kienböck hastalığında cerrahi tedavi sonuçlarımız

Öz

Kienböck hastalığı, tanı ve tedavisi zor bir hastalık olarak günümüzde önemini korumaktadır. Tedavi edilmediği takdirde elbileği instabilitesi, karpal kollaps, dejeneratif artroza yol açan sakatlayıcı bir hastalıktır. Lunatumdaki kollaps ve yapısal bozulma progressif bir seyir göstermektedir. Bu çalışma ile özellikle evre III tedavisindeki sorunlara ışık tutulmak istenmiştir. 1987-1997 yılları arasında El ve Mikrocerrahi Hastanesi'nde opere edilen 22 Kienböck hastası bu çalışmaya katılmıştır. Hastaların 11'i evre IIIb, 8'i evre IIIa 2'si evre II, 1’i evre I dir. Ortalama yas 26 ortalama rakip süresi 35 aydır. Kontrollerde kavrama gücü, eklem hareket genişliği, ağrı yakınması araştırılmış. Radyolojik olarak Stahl indeksi, karpal yükseklik ölçümü, ulnar "variance" ölçümü yapılmıştır. Olguların 11'i radyal kısaltma, 6'sı STT füzyon, 3'ü CH füzyon, 1'i proksimal dizi karpektomi, 1'i de drilleme eksternal fiksatör ile tedavi edilmiştir. İnterkarpal füzyon hastalarında hareket (%37.5) ve kavrama gücü (%55) azalması belirgindir. Radyal kısaltma osteotomisi yapılan olgularda kavrama gücü %72. hareket genişliği 7 hastada %82, 4 hastada % 65 seviyesindedir. Lichtman skorlamasına göre 1 hastada kötü (%5), 3 hastada orta (%15), 11 hastada iyi (%55), 5 hastada çok iyi (%25) sonuç elde edilmiştir. Sonuç olarak, radyal kısaltma sonuçlarımız, literatür ile uyumlu olmakla beraber interkarpal füzyon olgularında daha az tatminkar sonuçlar elde edilmiştir. İnterkarpal füzyonların teknik zorluğu, eklem içi girişimler olduğu ve deneyim gerektirdiği unutulmamalıdır. Kanımızca, evre IlIb'de seçilmiş olgularda radyal kısaltma uygulanabilir. İnterkarpal füzyon ise, tekniğine çok dikkat ederek ve artritik değişiklikler olmadığını BT ile kesin olarak ikna olduktan sonra uygulanmalıdır.
