Long-term results of neurovascular island flaps in thumb injuries

Abstract

Objectives: Neurovascular island flap may be used to cover soft tissue defects, in the restoration of sensibility, or for the reconstruction of the thumb. Cross sensibility and late orientation of sensibility of the thumb are the main problems associated with the use of this flap. This study evaluated sensibility and functions of the thumb in eight patients (7 males, 1 female; mean age 28 years; range 15-46 years) undergoing reconstruction with neurovascular island flap following thumb injuries. Methods: Eight patients underwent reconstruction with neurovascular island flaps for closure of soft tissue defect and simultaneously restoration of sensibility (n=5), stump revision (n=2), and secondary sensibility reconstruction (n=1). Sensibility assessments were made using the Semmes-Weinstein monofilament test, static and moving two-point discrimination tests (n=6), and cold intolerance. Mean follow-up was 37 months (range 30-45 months). Results: No flap losses were noted. Semmes-Weinstein monofilament test results were 2.83 in four thumbs, 3.61 in three thumbs, and 4.56 in one thumb. Static and moving two-point discrimination tests performed in six thumbs showed 4-10 mm and 6-15 mm, respectively. Cold intolerance was slight in three patients, mild in three patients, and severe in one patient. Conclusion: Although cross sensibility continued for a long time, it did not affect hand function, nor did it lead to function loss.
