Long-term results of neurovascular island flaps in thumb injuries

Objectives: Neurovascular island flap may be used to cover soft tissue defects, in the restoration of sensibility, or for the reconstruction of the thumb. Cross sensibility and late orientation of sensibility of the thumb are the main problems associated with the use of this flap. This study evaluated sensibility and functions of the thumb in eight patients (7 males, 1 female; mean age 28 years; range 15-46 years) undergoing reconstruction with neurovascular island flap following thumb injuries. Methods: Eight patients underwent reconstruction with neurovascular island flaps for closure of soft tissue defect and simultaneously restoration of sensibility (n=5), stump revision (n=2), and secondary sensibility reconstruction (n=1). Sensibility assessments were made using the Semmes-Weinstein monofilament test, static and moving two-point discrimination tests (n=6), and cold intolerance. Mean follow-up was 37 months (range 30-45 months). Results: No flap losses were noted. Semmes-Weinstein monofilament test results were 2.83 in four thumbs, 3.61 in three thumbs, and 4.56 in one thumb. Static and moving two-point discrimination tests performed in six thumbs showed 4-10 mm and 6-15 mm, respectively. Cold intolerance was slight in three patients, mild in three patients, and severe in one patient. Conclusion: Although cross sensibility continued for a long time, it did not affect hand function, nor did it lead to function loss.

Başparmak yaralanmalarında kullanılan damar ve sinirli ada fleplerinin uzun dönem sonuçları

Amaç: Başparmakta yumuşak doku kayıplarının örtülmesi, duyunun tekrar kazanılması ve başparmak rekonstrüksiyonları amacıyla damar ve sinirli ada flebi kullanılmaktadır. Flep uygulanmasından sonra çapraz duyarlılık ve duyunun başparmakta geç oryantasyonu, bu flebin problemleri olarak karşımıza çıkar. Bu çalışmada başparmak yaralanması nedeniyle damar ve sinirli ada flebi uygulanan sekiz olgunun (7 erkek, 1 kadın; ort. yaş 28; yaş dağılımı 15-46) başparmak duyu ve fonksiyonları değerlendirildi. Çalışma planı: Başparmak yaralanması sonucu oluşan yumuşak doku defektini kapatmak ve aynı anda duyuyu sağlamak (n=5), güdük rekonstrüksiyonu yapmak (n=2) ve sekonder duyu restorasyonu sağlamak (n=1) amacıyla damar ve sinirli ada flebi uygulandı. Duyu değerlendirmeleri Semmes-Weinstein monofilaman testi, hareketli ve statik iki nokta ayırım testleri (n=6) ve soğuk intoleransı ile yapıldı. Hastalar ortalama 37 ay izlendi (dağılım 30-45 ay). Sonuçlar: Hiçbir hastada flep kaybı olmadı. Semmes-Weinstein monofilaman testi ile dört parmakta 2.83, üç parmakta 3.61 ve bir parmakta 4.56 sonucu elde edildi. Altı parmakta yapılan hareketli ve statik iki nokta ayırım testlerinde sırasıyla 4-10 mm ve 6-15 mm arasında sonuçlar elde edildi. Üç hastada hafif, üç hastada orta, bir hastada şiddetli soğuk intoleransı görüldü. Çıkarımlar: Duyudaki çapraz lokalizasyon uzun süre devam etmesine rağmen, bunun fonksiyonları etkilemediği ve elde fonksiyon kaybına neden olmadığı görüldü.
