Keller resection arthroplasty applied in the treatment of hallux valgus deformity

Abstract

The surgical tecniques in the treatment of hallux valgus which can be corrected more than 130 different surgical procedures may be divided in two groups:
A.Soft tissue procedures,
B. B. Combined operations together with soft tissue procedure and osteotomies.
One of these in the second group operations is Keller's resection arthroplasty first used in 1904. 151 feet of 84 patients with hallux valgus deformity were treated by Keller's resection arthroplasty between the 1981 and 1990 in the orthopaedic department of Gülhane Military Medical Academy. A verage follow-up period was 13 months. 52 patients out of 84 cases were women while the other 32 were men. (A verage age was 63), 17 cases unilateral'y were operated. Our cases were evaluated both clinically and radiologically in the pre-op and post-op period. Patients older than 50 years old with moderate and severe hallux valgus deformity, who are treated by other tecniques are compared with our results. As a conclusion, this tecnique procedure provides a radical treatment and early mobilization of the patients 50 that it reduces the complication risks related to the older age,

Halluks valgus deformitesinde Keller rezeksiyon artroplastisi

Abstract

130'dan fazla cerrahi yöntemle tedavisi yapılabilen halluks valgus'taki cerrahi yöntemler başlıca iki grupta toplanabilir;
A. Yumuşak doku gevşetmeleri.
B. B. Yumuşak doku gevşetmeleri ile birlikte yapılan osteotomilerden oluşan kombine ameliyatlar.
Bu ikinci grup ameliyatlardan birisi de ilk defa 1904 yılında KELLER tarafından bildirilen rezeksiyon artroplastisidir. Gülhane Askeri Tıp Akademisi Ortopedi ve Travmatoloji Anabilim Dalı'nda 1981-1990 yılları toplam 84 Halluks Valgus'lu hastanın 151 ayağına Keller artroplastisini uyguladık. Ortalama takip süremiz 13 ay olup vakalarımızın 52 tanesi kadın, 32 tanesi erkekti.
Hastalarımızın yaş ortalaması 63'tür. 67 vakaya operasyon bilateral 17 vakaya ise tek taraflı uygulandı.Olgularımızın preoperatif ve postoperatif dönemde klinik ve radyolojik olarak değerlendirildi. Sonuçlarımız 50 yaşın üstünde orta ve ileri derecedeki Halluks Valgus'lularda uygulanmış olan diğer yöntemler ile karşılaştırıldı. Sonuç olarak Keller ameliyatı hastalarda radikal tedavi ve erken mobilizasyon sağlayarak yaşlılığa baglı komplikasyon risklerini azaltır.
