Femoral neck fractures treated with cannulated screw

Abstract

We reviewed the results in 22 patients with femoral neck fractures who were managed with cannulated screw fixation in Ankara University İbn-i Sina Hospital between January 1996 and December 1997. There were 14 male and 8 female patients with a mean age of 36.5 years. Most of them suffered high energy fractures, the leading cause was motor vehicular accidents. 2 patients had a Garden Stage II fracture, 12 Stage III and 8 Stage IV. The patients were treated within a mean of two days after injury with 3 parallel cannulated screws in all but two. Our mean follow- up is 23 months (Range:13-32 mo.) Avascular necrosis of the femoral head developed in 3 patients and pseudoarthrosis in 2 patients. We think immediate reduction and parallel cannulated screw fixation, femoral neck fractures can be managed effectively with low morbidity.

Abstract

Bu makalede, kliniğimizde Ocak 1996 - Aralık 1997 yılları arasında kanüle vida fiksasyonu ile tedavi edilen toplam 22 femur boyun kırıklı hastanın sonuçları incelenmiştir. Hastaların 14'ü erkek, 8'i kadın olup yaş ortalamaları 36.5 idi. Çoğunda yüksek enerjili travma sonucu kırık oluşmuştu (% 81.8 trafik kazası) . Garden sınıflamasına göre 2 hasta Tip II, 12 hasta Tip III ve 8 hasta ise Tip IV olarak degerlendirildi. Hastalar ortalama 2 gün sonra ameliyata alınarak ikisi dışında hepsi 3 adet paralel kanüle spongioz vida ile fikse edildi. Ortalama takip süremiz 23 ay olup 13-32 aylar arasında değişmektedir. Takipte 3 hastada avasküler nekroz, 2 hastada nonunion gelişmiş, 17 hasta ise sorunsuz iyileşmiştir. Bu sonuçlar ışığında dikkat ile seçilmiş hasta gruplarında kanüle vidalar ile kollum femoris kırığı tedavisinin güvenilir olduğu düşüncesindeyiz.
