The effect of tolmetin on tendon healing (A biomechanical study on rabbits)

Abstract

Introduction: The use of NSAIDs are common after musculoskeletal injury or surgeries. The influence of NSAID on tendon healing is controversial. We evaluated the effect of tolmetin on tendon healing. Material and Method: 12 Male New Zealand rabbits randomly selected and tenotomy of the right achilles tendon and cast immobilization in flexion was performed. While 6 rabbits in experimental group receiving 25 mg/kg/day tolmetin via intraperitoneal injection other 6 rabbits consisted the control group and not given tolmetin. All the rabbits were sacrified and achilles tendons were resected at 14 th day. On material testing machine biomechanical evaluation was made. Average tensile force-to-failure was 33.81 newton (N) for the experimental group and 36.95 N for the control group. No statistically significant difference between the groups was found. Discussion: Because the first stage of tendon healing is inflammation, one may expect that while NSAIDs reducing pain and swelling thus providing clinical benefits on the other hand they may interfere with tendon healing. We found that tolmetin has no adverse effect on healing of conservatively treated achilles tendon rupture. Conclusion: During tendon healing tolmetin and probably other similar NSAIDs may be useful since these drugs can reduce adhesions caused by surgery to minimum and increase gliding function of tendon thus an optimum clinical result can be obtained.

Tolmetinin tenden ivilesmesine etkisi (Tavşanlarda biyomekanik bir çalışma)

Abstract

Non-steroidal antiinflamatuar ilaç olan tolmetinin tendon iyileşmesine etkisinin araştırıldığı çalışmada 12 Yeni Zelanda cinsi tavşanın sağ aşil tendonuna perkütan tenotomi yapıldı. Tendon uçlarını yaklaştırmak için, alt ekstremite diz ve ayak bileği eklemi 45 derece fleksiyonda fiberglas ile hareketsiz hale getirildi. Altı tavşan rastgele seçilerek deney grubunu oluşturdu. Bunlara günde 25 mg/kg tolmetin intraperitoneal enjeksiyon yoluyla verildi. Diğer altı tavşan kontrol grubunu oluşturdu ve bunlara sadece serum fizyolojik verildi. 14. günün sonunda denekler öldürulerek aşil tendonları çıkartıldı. Bütün örneklere aynı çevre şartları altında biyomekanik test uygulandı. Dakikada 5 mm hızla instron cihazında tek yönlü çekme uygulanarak her bir örneğin gerilim kopma noktası belirlendi. İlaç verilen grupta ortalama kopma kuvveti 33.81 N (newton) iken kontrol grubunda bu değer 36.95 N olarak saptandı. İki grup arasında istatistiki açıdan anlamlı bir fark bulunmadı. Sonuç olarak, tolmetinin tendon iyileşmesinin erken evresinde verilmesinin iyileşmeyi olumsuz etkilemediği kanısına varıldı.
