Hematopoetik Kök Hücre Nakillerinde HLA Doku Tiplemesinin Önemi

Abstract

Hematopoetik kök hücre nakli (HKHN), malign ve malign olmayan hematolojik hastalıklarda iyileştirici bir tedavi seçeneğidir. Alıcı-verici arasındaki HLA uyumu, cinsiyet uyumu nakil sonrası hasta ve greft sağkalımını etkileyen önemli parametreler arasındadır. Nakiller çoğunlukla kardeş vericilerle sınırlı olmasına rağmen, günümüzde HLA uyumlu akraba dışı vericilerden yapılan nakiller son yıllarda giderek artmıştır. HLA tipleme, alıcı-verici HLA-A, -B ve -DR için fenotipik uyumunu saptayabilen serolojik ve hücresel yöntemlerle gerçekleştirilmektedir. DNA temelli tiplemenin sağladığı artan çözme gücü, akraba içi uyumlu verici yokluğunda veya akraba dışı vericilerin tiplemesinde, daha iyi uyumlu vericilerin tanımlanmasına olanak tanımaktadır. Yüksek çözünürlüklü DNA temelli HLA tipleme yöntemlerinin geliştirilmesi, HLA uyumlu akraba dışı vericilerin tanımlanmasını ve buna bağlı olarak nakil başarı şansını önemli derecede arttırmıştır.
