INVESTIGATING THE EFFECTS OF IMMEDIATE PROSTHETIC FITTING

Abstract

This study is done with the aim of investigating the effects of immediate prosthetic fitting technique. A total of 26 patients between 21 and 55 ages and whose amputation causes were trauma and cancer and diabetes mellitus participated in the study. The patients were assigned into two groups. In the group who were provided with immediate prostheses, there were 11 patients (8 transtibial, 3 trans-femoral) while there were 15 patients (11 trans-tibial, 4 trans-femoral) who were provided with classical prosthetic application. They were all assessed for their muscle strength, range of motion, anthropometric measurements, phantom pain and sense, weight bearing through the amputated limb, gait, quality of life, and prosthetic training period. There were significant differences between the groups when the anthropometric values, data of weight bearing, gait, quality of life, phantom sense and pain and prosthetic training were compared (p<0.05). It can be stated that the quality of life of the amputees will be improved by the early ambulation facility in immediate prostheses fitting technique this condition will also shortenes the period of transition to definite prostheses by shaping the stump in a shorter period of time. It can be concluded that by the immediate prosthetic fitting technique the amputees can therefore reintegrate to normal living in a shorter period

ERKEN PROTEZ UYGULAMALARININ ETKİLERİNİN ARAŞTIRILMASI

Abstract

Bu çalışma alt ekstremite amputelerinde erken protez (immediate protez) uygulamalarının etkilerinin araştırılması amacıyla planlandı. Çalışmaya yaşları 21-55 yıl arasında değişen, amputasyon nedeni travma, tümör ve diabetes mellitus olan toplam 26 olgu alındı. Olgular 2 gruba ayrıldı. Erken protez uygulanan grupta 8’i diz altı 3’ü diz üstü olmak üzere 11 olgu yer alırken, klasik uygulama yapılan grupta 11’i diz altı, 4’ü diz üstü olmak üzere toplam 15 olgu yer aldı. Olgular fiziksel özellikleri, kas kuvveti, normal eklem hareketleri, antropometrik ölçümler (güdük boyu ve çevre ölçümleri), fantom hissi ve ağrısı, ampute taraf ağırlık aktarma, yürüyüş, yaşam kalitesi ve protez eğitim süresi açısından değerlendirildi. Elde edilen veriler gruplara göre karşılaştırıldığında, güdük çevre ölçümleri, ampute taraf ağırlık aktarma, yürüyüş, yaşam kalitesi, fantom hissi - ağrısı ve protez eğitim süresi açısından erken protez uygulanan grup lehine anlamlı sonuçlar elde edildi (p<0.05). Alt ekstremite amputelerinin rehabilitasyonunda önemli bir yere sahip olan erken protez uygulamaları ile daha erken ambulasyonun gerçekleştiği, buna bağlı olarak güdüğün kısa sürede şekillenme sürecini tamamladığı ve kalıcı proteze geçişin hızlandığı, amputenin normal yaşantısına mümkün olan en kısa sürede geri dönüşünün sağlandığı gösterildi.
