POSTPARTUM VULVAR PEDUNKÜLE CELLÜLER ANJİOFİBROM: OLGU SUNUMU

Abstract

Cellüler anjiofibrom vulvanın nadir görülen yavaş büyüyen benign mezenkimal tümörüdür. Klinik semptom genelde yoktur, premenopozal yaş grubunda daha sık tespit edilir.Vulvar bölgede daha sık izlenir. Genellikle bu benign tümörlerde stroma invazyonu görülmez ve tedavide basit lokal eksizyon yeterlidir. Kadın ve erkeklerde eşit oranda görülmekle birlikte ; gerçek insidansı net değildir 46 yaşında kadın hasta, yaklaşık 8 cm boyutlarında, iyi sınırlı, belirgin, solid, vulvada sağa deviye şekilde ,saplı kitle şikayeti ile başvurdu. Hasta öyküsünde kitlenin 3. vajinal doğumdan hemen sonra çıktığını ,adetle büyüyüp, adet bitiminde küçüldüğünü ifade etti. MR sonucu; mevcut kitlenin düzgün sınırlı ,solid ,IVKM sonrası yoğun kontrast tutulumu gösterdiği tespit edildi. Immünohistokimyasal olarak CD34:vascüler yapılar (+) ,Düz kas Aktin :fokal + tespit edildi . S100 ,ki67 %1 (+),CD31: vascüler yapılar (+) idi. ER ve PR (+) idi.Vulvavajinal cellüler anjiofibroma patofizyolojisinin daha net anlaşılması yeni tedavi rejimlerine alternatif sunacaktır.

POSTPARTUM PEDUNCULATED ANGIOFIBROMA OF THE VULVA: CASE REPORT

Abstract

Cellular angiofibroma is a rare, slow-growing benign mesenchymal tumor of the vulva. There are no clinical symptoms in general, it is detected more frequently in the premenopausal age group. It is observed more frequently in the vulvar region. Stroma invasion is not usually seen in these benign tumors and simple local excision is sufficient for treatment. Although it is seen equally in men and women; the true incidence is not clear. A 46-year-old female patient presented with a well-circumscribed, prominent, solid, vulva deviated to the right pedunculated mass of approximately 8 cm. In her history, the patient stated that the mass appeared after the third vaginal delivery, enlarged with menstruation, and decreased at the end of menstruation. MRI result; determined that the present mass was well-defined, solid, and showed intense contrast enhancement after IVCM. Immunohistochemically CD34: vascular structures (+), Smooth muscle Actin: focal + were detected. S100, ki67 1% (+), CD31: vascular structures were (+). ER and PR were (+). The clearer pathophysiology of vulvovaginal angiofibroma will offer an alternative to new treatment regimens.
