Comparative İnvestigation of Biochemical and Physiological Changes İn The Minor Surgery Of Geriatric And Other Age-Classified Patients

Abstract

Aim: The aim of this study was to examine the effects of advancing age and body composition changes on the metabolic response to minor oral surgical operations of geriatric patient. Acute phase proteins such as plasma fibrinogen, C-reactive protein (CRP) and serum albumin levels were investigated and compared with other aged patients. Materials and Methods: The study was carried out on 32 subjects whom are attended for minor surgery to department. Patients were divided into two age groups as: Age under 65 was the first group, age of 65 and older was the second group. Samples of blood were taken before the operation, 2 day and 7 day after the operation, respectively. Plazma fibrinogen, serum albumin and CRP levels were investigated. Statistical analysis were carried out with Student-T test for intra-group values and Anova between different group values. Results: There were no significant difference found between two groups according to plasma fibrinogen and serum albumin values when compared before, after two day and 1 week the operation, respectively (p>0.05). Also there were no significant effect of age on values (p>0.05). Altough CRP values were increased slightly 2 day after the operation, these values were found in the avarage range of this protein. Conclusion: From this study it is concluded that there is no effect of advancing age and body composition changes on the metabolic response to minor oral surgical operations of geriatric patients.

Geriatrik ve Diğer Yaş Gruplarında ki Hastalarda Uygulanan Minör Cerrahi Girişimlerde Meydana Gelen Biyokimyasal ve Fizyolojik Değişikliklerin Karşılaştırmalı İncelenmesi

Abstract

Amaç: Bu çalışmanın amacı geriatrik hastalarda, ilerlemiş yaş ve vücut kompozisyonu değişikliklerinin minör oral cerrahi girişimlerden sonra meydana gelen metabolizma cevabına etkilerini araştırmaktır. Bu amaçla akut faz cevabı proteinlerinden olan plazma fıbrinojen, C-reaktif protein (CRP) ve serum albumin seviyelerindeki değişimler takip edilerek diğer yaş grubuyla kıyaslanmıştır. Yöntem ve Gereç: Çalışma, kliniğimize minör cerrahi operasyon için başvuran 32 hasta üzerinde gerçekleştirilmiştir. Hastalar; 65 yaş altı erişkin grup (1. grup) ve 65 yaş ve üstü geriatrik grup (2.grup) olarak ikiye ayrılmıştır. Hastalardan operasyon öncesi, operasyon sonrası 2. ve 7. günde kan örnekleri alınarak plazma fibrinojen, serum albumin ve CRP konsantrasyon değerleri belirlenmiştir. İstatistiksel analizler, grup içi değerler için student-t testi, farklı gruplar arasındaki değerler için ise anova testi ile gerçekleştirilmiştir. Bulgular: Operasyon öncesi ile sonrasında ki 2. ve 7. günde elde edilen değerler karşılaştırıldığında, plazma fibrinojen ve serum albumin seviyelerinde anlamlı bir değişiklik görülmemiştir (p>0.05). Yaşa göre yapılan karşılaştırılmalarda da anlamlı sonuç tespit edilememiştir (p>0.05). Operasyonun 2. günü her iki grupta serum CRP değerlerinde meydana gelen benzer oranlardaki artış olsa da, bu değerler normal kabul edilen sınırlar içerisinde bulunmuştur. Sonuç: Sonuç olarak geriatrik hastalarda ilerlemiş yaş ve vücut kompozisyonu değişikliklerinin minör oral cerrahi girişimlerden sonra meydana gelen metabolizma cevabına ve akut faz reaktanlarından plazma fibrinojen, serum CRP ile albumin üzerinde herhangi bir etkisi olmadığı belirlenmiştir.
