TREATMENT OF AN ANTERİOR OPEN BITE WITH THE KİM’S TECHNIQUE MODIFIED BY ENACAR AND AFTER TWO YEARS POST-TREATMENT FOLLOW-UP

Abstract

A malocclusion characterized by open-bite can be one of the most difficult problems to correct through orthodontic treatment because of the difficulty morphological disharmony and functional disorder associated with open-bite. The orthodontic treatment of anterior open bite malocclusion requires different treatment methods during different growth stages. One of the methods for the treatment of open bite is the multiloop edgewise arch wire technique developed by Kim. This technique comprises the use of multiloop gable bend arch wires
with vertical elastics in the canine regions. The aims of this technique involve correction of the inclination of the occlusal planes and uprighting of the axial inclinations of the posterior teeth. Enacar et al modified Kim’s technique by using 0.016 x 0.022 inch upper accentuated-curve and lower reverse-curve nickel titanium arch wires in place of multiloop wires with the intermaxillary elastics applied in the canine areas. In this study was to evaluate effects of open bite treatment with Kim’s technique modified by Enacar on a patient with anterior open bite and 2-year follow-up after treatment is presented. A 15-years-old female patient’s main complaint was not to be able to bite with her anterior teeth. The patient had a skeletal and dental class I relationship, and high angle characteristics with an ANB 3,5 degrees, FMA of 36,7 degrees, 5 mm of overjet and - 4 mm overbite according the clinical and radiographic examinations. The patient received conservative orthodontic treatment without tooth extraction. After 16 months of orthodontic treatment achieved class I molar and canine relationship and 2 mm overbite. Bite closure was achieved by uprighting and extrusion of upper and lower incisors. Openbite activator and Hawley appliance were used by the patient during one year for retention. Satisfactory results were observed after two years folow-up.

BİR AÇIK KAPANIŞ OLGUSUNUN ENACAR MODİFİYE KİM TEKNİĞİ İLE TEDAVİSİ VE TEDAVİ SONRASI 2 YILLIK TAKİBİ

Abstract

Açık kapanışla karakterize maloklüzyonlar, açık kapanışa neden olan fonksiyonel bozukluklar ve morfolojik düzensizlikler nedeniyle ortodontik açıdan tedavisi güç durumlardandır. Ön açık kapanış malokluzyonunun ortodontik tedavisi amacıyla büyüme ve gelişimin farklı dönemlerinde farklı tedavi yöntemleri kullanılmaktadır. Bu yöntemlerden biri de Kim tarafından geliştirilen multi loop’lu edgewise tekniğidir. Bu teknik multiloop çatı bükümleri ve kanin dişleri bölgesinde dikey elastiklerin kullanımını içermektedir. Bu tekniğin kullanımı ile amaçlanan ise posterior dişlerin dikleştirilmesi ve oklüzal düzlem eğiminin düzeltilmesidir. Enacar ve arkadaşları, multi loop bükümleri yerine 0.016 x 0.022 inch kalınlığında üst artttırılmış spee’li ve alt ters spee’li nikel titanyum teller ve kaninler bölgesinde intermaksiller elastikler kullanarak Kim metodunu modifiye etmişlerdir. Bu çalışmada, ön açık kapanışa sahip bir hastanın Enacar modifiye Kim tekniği kullanılarak yapılan tedavisi ve hastanın tedavi sonrası 2 yıllık takibi sunulmuştur. 15 yaşındaki bayan hasta kliniğimize ısıramama şikâyeti ile başvurdu. Klinik ve radyografik incelemeler sonucunda; 5 mm. overjete ve - 4 mm. overbite’a sahip olan hastanın 3,5 derecelik ANB, 36,7 derecelik FMA açısıyla karakterize dik yön boyutları artmış iskeletsel sınıf I vaka olduğu tespit edildi. Hastaya çekimsiz ortodontik tedavi yapılmıştır. 16 aylık ortodontik tedavi sonrasında sınıf I molar ve kanin ilişkisi ve 2 mm. overbite elde edilmiştir. Açık kapanış, alt ve üst kesici dişlerin uzaması ve dikleşmesi ile kapatılmıştır. Hasta pekiştirme apareyi olarak openbite aktivatörü ve Hawley plağı kullanmıştır. İki yıllık takip sonucunda ise tatmin edici sonuçlar izlenmiştir
