DİŞ HEKİMLERİNİN PERİAPİKAL RADYOGRAFİLER VE KONİK IŞINLI BİLGİSAYARLI TOMOGRAFİ (CBCT) GÖRÜNTÜLERİNE GÖRE UYGULAYACAKLARI ENDODONTİK TEDAVİ SEÇENEKLERİNİN KARŞILAŞTIRILMASI

Amaç: Çalışmamızın amacı ağız diş ve çene cerrahisi, periodontoloji, endodonti, protetik diş tedavisi hekimleri ile diş hekimlerinin endodontik tedavi gerektiren hastalarda uygulayacakları tedavi planlarının periapikal radyografiye ve konik ışınlı bilgisayarlı tomografiye göre göstermiş olduğu değişikliklerin araştırılmasıdır. Gereç ve Yöntem: Çalışmadaki sekiz vaka mevcut kayıtlı hasta arşivinden elde edildi. Anket çalışmamızda seçilen hastalar için oluşturulan vaka tanımları her iki görüntüleme yöntemiyle ayrı ayrı ve iki aşama arasında en az 4 hafta sonra olacak şekilde 305 hekime uygulandı. Hekimlerin her iki aşamada da 8 vaka için ayrı ayrı belirlenen tedavi planlamalarından en uygun olanını ve bu aşamadaki karar verme zorluğunu seçmeleri istendi. İstatistiksel analiz için tanımlayıcı istatistiksel ölçütler, bağımsız örneklem t test, tek yönlü varyans analizi, Likelihood Ratio, Mann Whitney U, ve Kruskal Wallis H testi kullanıldı. Bulgular: Çalışmamıza dahil edilen 305 hekimin; 111’ini diş hekimleri, 46’sını ağız diş ve çene cerrahisi hekimleri, 46’sını periodontoloji hekimleri, 49’unu endodonti hekimleri ve 53’ünü de protetik diş tedavisi hekimleri oluşturmaktadır. Uzmanlık alanları arasında tedavi planlamasında farklılıklar olduğu ve genellikle endodonti hekimlerinin diğer hekimlere göre tedavi planlamasını her iki aşamada da daha konservatif yaptığı ve tedavi planlama aşamasındaki zorluk derecesi algılarının diğer hekimlere göre daha düşük olduğu belirlenmiştir. Sonuç: Çalışmamızda hekimlerin CBCT görüntüleri ile yaptıkları planlamalarda özellikle komplike vakalar açısından farklılıklar meydana geldiği görülerek tedavi planlamasında uzmanlık alanının önemi anlaşılmıştır. COMPARISON OF ENDODONTIC TREATMENT OPTIONS THAT DENTISTS APPLY ACCORDING TO PERIAPICAL RADIOGRAPHS AND CONE-BEAM COMPUTED TOMOGRAPHY (CBCT) IMAGES ABSTRACT Aim: The objective of our study is to conduct research on changes in the treatment plans to be applied by oral and maxillofacial surgeons, periodontists, endodontists, prosthetic dentists, and dentists on patients requiring endodontic treatment according to periapical radiography and cone-beam computed tomography. Materials and Methods: The eight cases in the study were obtained from the available registered patient archive. The case definitions created for the selected patients in our survey study were applied to 305 physicians via both imaging methods separately and by keeping minimum 4 weeks between the two stages. The physicians were requested to select the most appropriate one of the treatment plans determined separately for the 8 cases at both stages and the difficulty in decision-making at this stage. For statistical analysis, descriptive statistical criteria, independent samples t-test, one-way analysis of variance, the Likelihood Ratio, Mann-Whitney U and Kruskal-Wallis H tests were used. Results: Of the 305 physicians included in our study, 111 are dentists, 46 are oral and maxillofacial surgeons, 46 are periodontists, 49 are endodontists, and 53 are prosthetic dentists. It was found that there were differences in treatment plans among the specialty, endodontists generally planned the treatment more conservatively than other physicians at both stages, and their perceptions of the difficulty level at the stage of treatment planning were lower compared to other physicians. Conclusion: It was observed in our study that differences occurred especially in the treatment plans, which were made by physicians with CBCT images, for complicated cases, and the importance of the dental specialty status was understood in the decisions.
