SINIF II BÖLÜM 1 MALOKLÜZYONLU BİREYLERDE FONKSİYONEL ORTOPEDİK TEDAVİ ETKİLERİNİN AKSİYOGRAFİK VE SEFALOMETRİK YÖNTEMLERLE İNCELENMESİ

Abstract

Amaç: Bu çalışmada, Sınıf II bölüm 1 maloklüzyonlu bireylerde aktivatör ile yapılan fonksiyonel ortopedik tedavinin, kranio-fasiyal yapılarda meydana getirdiği değişikliklerin incelenmesi ve temporomandibuler eklem fonksiyonları ve hareket biyomekaniği üzerine etkilerinin belirlenmesi amaçlandı. Gereç ve Yöntem: İskeletsel Sınıf II bölüm 1 maloklüzyona disfonksiyonu belirtisi göstermeyen ve pubertal büyüme atılımı başlamış, kronolojik yaş ortalamaları 11.2 yıl ± 9 ay olan 23 kız ve 12.9 yıl ± 8 ay olan 21 erkek birey incelendi. Araştırma kapsamına alınan bireyler, 24 kişilik tedavi ve 20 kişilik kontrol gruplarına ayrıldı ve 10 ay süresince değerlendirildi. Araştırmamızın materyalini, bu bireylerden uygulama öncesi ve sonrası elde edilen lateral sefalometrik filmler ve uygulama öncesi, 2. ay, 4. ay, 6. ay, 8. ay ve uygulama sonunda alınan aksiyografik kondiler hareket kayıtları oluşturdu. Lateral sefalometrik filmler üzerinde iskeletsel, dento-alveoler ve yumuşak doku ölçümleri yapılırken, aksiyografik kondiler hareket kayıtları üzerinde açma-kapama ve protrüzyon uzunluğu, açma-kapama ve protrüzyon açısı, açma-kapama ve protrüzyon derinliği ölçümleri yapıldı. Hastalardan ayrıca maksimum ağız açma mesafesi kaydı alındı. Bulgular: Lateral sefalometrik filmler üzerinde yapılan incelemelerde, tedavi ve kontrol grupları arasında çeneler arasındaki ilişkilerin düzelmesine veya büyüme etkilerine bağlı olarak istatistiksel olarak anlamlı farklılıkların ortaya çıktığı belirlendi. Aksiyografik veriler ve maksimum ağız hareket ölçümleri incelendiğinde ise, tedavi grubunda zaman faktörü ile ilişkili olarak ilgili yapılarda tedaviye bağlı ortaya çıkan değişimler sonucunda yine istatistiksel olarak anlamlı değişiklerin meydana geldiği tespit edildi. Sonuç: Aktivatör ile yapılan fonksiyonel tedavi sonucunda iskeletsel, dento-alveoler ve yumuşak dokularda meydana gelen değişimlere bağlı olarak çeneler arası ilişkinin düzeldiği ve temporomandibulereklem fonksiyonlarında adaptif değişikliklerin ortaya çıktığı belirlenmiştir
