FARKLI TEMPOROMANDİBULAR EKLEM DÜZENSİZLİĞİNE SAHİP HASTALARDA UYGULANAN ARTROSENTEZ İŞLEMLERİNİN SAYILARININ RETROSPEKTİF OLARAK İNCELENMESİ

Abstract

Amaç: Bu çalışmanın amacı farklı temporomandibular eklem düzensizliklerine sahip bireylerde uygulanan artrosentez sayılarının ve seanslar arası sürelerin incelenerek, eklem hastalığı tipine göre uygulanacak tedavi protokolü hakkında öngörü sağlayabilmektir.
Gereç ve Yöntem: Bu retrospektif çalışma, “Temporomandibular Düzensizlikler için Tanı Kriterleri” sınıflamasına göre, redüksiyonlu disk deplasmanı, redüksiyonsuz disk deplasmanı ve dejeneratif eklem hastalığı tanısı konulan ve kliniğimizde artrosentez tedavisi gören 160 hastaya uygulanan artrosentez sayısını ve sıklığını incelemektedir. İki yıllık tedavi ve takip süresi boyunca hastalara uygulanan artrosentez sayıları ve birden fazla artrosentez uygulanan hastalarda her seans arası süreler hesaplanarak analiz edilmiştir.
Bulgular: Redüksiyonlu disk deplasmanı, redüksiyonsuz disk deplasmanı ve dejeneratif eklem hastalığı gruplarında medyan seans sayıları karşılaştırıldığında gruplar arasında istatistiksel olarak anlamlı farklılığa rastlanıldı (p<0.001). Birinci ve ikinci seanslar arasındaki süre hem redüksiyonsuz disk deplasmanı hem de dejeneratif eklem hastalığı gruplarında benzer iken (p = 0.386), ikinci ve üçüncü seanslar arasındaki süre redüksiyonsuz disk deplasmanı grubuna göre dejeneratif eklem hastalığı grubunda anlamlı olarak daha uzun bulundu (p = 0.002).
Sonuç: Temporomandibular eklem düzensizliklerinin seviyesi ilerledikçe ihtiyaç duyulan artrosentez sayısı artmaktadır. Seanslar arası süre ise temporomandibular eklem düzensizliğinin tipine göre değişebilmektedir.

Retrospective analysis of the number of arthrocentesis procedures performed in patients with different temporomandibular joint disorders
ABSTRACT
Aim: The aim of this study is to provide prediction about the treatment protocol according to the type of joint disease by examining the number and the duration between the sessions of arthrocentesis performed in individuals with different temporomandibular joint disorders.
Material and Method: This retrospective study investigated the number and frequency of arthrocentesis performed in 160 patients who were diagnosed with either reduced disc displacement (DDwR), disc displacement without reduction (DDwoR) or degenerative joint disease (DJD) according to the “Diagnostic Criteria of Temporomandibular Disorders (DC / TMD)”. During the 2-year treatment and follow-up period, the number of arthrocentesis procedures and the time between each session in patients who underwent more than one arthrocentesis were calculated and analyzed.
Results: A statistically significant difference was found between DDwR, DdwoR and DJD groups when the median session numbers were compared (p <0.001). While the time between the first and second sessions was similar in both the DDwoR and DJD groups (p = 0.386), the time between the second and third sessions was significantly longer in the DJD group compared to the DDwoR group (p = 0.002).
Conclusion: As the status of temporomandibular joint disorder progresses, the number of arthrocentesis needed increases. The time between sessions may vary depending on the type of temporomandibular joint disorder.
