Abstract

Günümüzdeki cerrahi gelişmeler ve literatürdeki bilgiler ışığında, sigmoid kolon volvulusunun tedavi ve mortalitesini etkileyen sorunlar eleştirilmiştir.
Ocak 1965 - Şubat 1975 arasında kliniğimizde ivedili tedavi gören 17 sigmoid kolon volvulusu vakası sunulmuştur. Rezeksiyon uygulanan 10 vakada mortalite % 40, basit detorsiyon, peksi veya plikasyon uygulanan 7 vakada ise % 14,3 olarak saptanmıştır. Sigmoidoskopik entübasyon hiçbir hastada uygulanmamış olmakla birlikte olumlu yönleriyle seçkin vakalarda yararlı olabileceği kanısına varılmıştır.
Laparatomide, nekroz saptanan vakalarda yüksek mortalite oranına karşın rezeksiyon veya Paul - Mikulicz eksteriorizasyonu uygulanması zorunludur. Barsak canlı ve normal görünümdeyse basit detorsiyon, peksi yaparak kesin tedaviyi ileride
uygulanacak seçkin rezeksiyona bırakmak hasta yaşamı yönünden olumlu bir karar olacaktır.

17 Sigmoid Kolon Volvulusu Üzerinde Klinik İnceleme ve Tedavi Yönteminin Seçimi

Abstract

In light of modern surgical development and literature in- formation, problems which affect sigmoid colon volvulus treat- ment and mortality are criticized.

17 sigmoid colon volvulus cases, who have had emergency treatment in our clinic, between Jan. 1965 and Feb. 1975, are prosented. Mortality rates were found as; 40 % in 10 resection cases 14,3 % in seven simple detorsion, pexy or pilication cases.

Although sigmoidoscopic intubation has never been performedin any of our cases, we believe that it can be advantageous in elective cases.

A high mortality rate is common, when faced with necrosis of the bowel, in laparatomy. Resection or Paul - Mikulicz type
of exteriorization is a must. If the gut is viable and normal in appearance, we recommend that simple detorsion and paxy be performed to save the patients life. When the patient has recovered for further surgical intervention, elective rezection
should be performed.
