Abstract
1 — Hemoroidektomi ile birlikte uygulanan internal sfinkteretomi operasyonunun postoperatif ağrı üzerindeki etkisini araştırmak amacıyla, 54'ünde sfinkteretomi uygulanan, 30'unda uygulanmayan toplam 84 olgu incelendi.
2 — Sfinkteretomi uygulanan 54 olgudan histolojik inceleme için sfinkter biyopsisi alındı. Bunlardan 33'ünden alınan biyopsilerin internal sfinkterete ait olduğu ve sadece bu olgularda internal sfinkteretominin kesin olarak uygulandığı görüldü. Aynı zamanda, bu biyopsi sonuçlarına dayanarak, hemoroid ve anal fissür operasyonlarında operasyon alanının tabanında gözlenen dairesel kas liflerinin, eksternal sfinkterin subkütanöz kısmı değil, internal sfinkterin en alt kısmı olduğu yönündeki yeni görüşü destekleyen kanıtlar elde edildi. 3 — Hemoroidektomi ile aynı anda internal sfinkteretomi uygulanan vakalarda, postoperatif ağrı açısından oldukça tatmin edici sonuçlar elde edilmiştir. İnternal sfinkteretomi ile bağlantılı olarak herhangi bir erken veya sonraki komplikasyon gözlenmemiştir. 4 — Herhangi bir sonradan düşünme olmaksızın, hemoroidektomi ile birlikte internal sfinkteretomi uygulamasının postoperatif ağrıyı önlemek amacıyla yararlı ve gerekli olduğu kanısına varılmıştır.

Abstract
1 — A total of 84 cases of which sphyncteretomy was carried
out in 54 and not carried out in 30, was examined for the purpose
of investigating the influence on post - operative pain of internal
sphyncteretomy operation carried out together with hemoroidectomy.
2 — Of the 54 cases in which sphynteretomy was carried out, sphyncter biopsy was taken out for histological investigation. It was
seen that the biopsies taken from 33 of these pertained to the internal
sphyneter, and that only in these cases was internal sphyncteretomy
definitely implemented. At the same time, on the basis of these
biopsy results, evidence was obtained to support the new view that
the circular muscle fibers observed at the base of the operational
field in hemorroid and anal fissure operations are not the subcuteneus
part ot the external sphyncter but the lowest portion of the internal
sphyncter.
3 — In cases where internal sphyncteretomy was carried out
at the same time as hemorroidectomy, very satisfactory results
were obtained from the point of postoperative pain. No immediate
and subseguent complications 'whatever were observed in connection
with the internal sphyncteretomy.
4 — Without any afterthoughts whatever the conviction was
reached that the implementation of internal sphyncteretomy With
hemorroidectomy is useful and necessary for the purpose of preventing
postoperative pain.
