Abstract

The problem-oriented medical system is a tool which provides a systematic approach to patient care by providing clear, concise, and structured statements about medical actions. The focal point of the system is the problem-oriented medical record. This record system enhances continuity of patient care and allows one to document outcome of patient care. The problem-oriented medical system allows all individuals involved in a patient’s care to determine the importance and relevance of their activities. Except for the timing and integration of various medical actions, the problem-oriented medical system does not dictate the type of information gathered about a patient or methods of problem management. The problem-oriented medical system is most useful to those who comprehend the concepts upon which it is base, and who are familiar with the mechanics of the problem-oriented medical record

Abstract

Probleme yönelik tıbbi sistem; tıbbi uygulama hakkında açık, özlü ve yapılandırılmış açıklamalar sağlayarak hasta bakımına sistematik bir yaklaşım sunan bir araçtır. Sistemin odak noktası probleme yönelik tıbbi kayıttır. Bu kayıt sistemi hasta bakımının devamlılığını sağlar ve bakımı yapan kişiye hastanın sonuçlarını kayıt altına alma fırsatı verir. Probleme yönelik tıbbi sistem, bir hastanın bakımı ile ilgili olan her bir kişiye kendi müdahalelerinin önemi ve uygunluğunu tespit etme şansını yaratır. Probleme yönelik tıbbi sistem çeşitli tıbbi müdahalelerin birleştirilmesi ve zamanlaması haricinde bir hastaya ilişkin toplanması gereken bilginin çeşidi veya eğer problemin yönetilmesi gerekiyorsa ona ilişkin metodu belirlemez. Probleme yönelik tıbbi sistem, gereken anlayışı kavrayabilen ve probleme yönelik tıbbi kayıtların mekanizmasını bilen kişiler için kullanışlıdır
