Impact of Clinical Prediagnosis on HRCT Sensitivity in Otosclerosis

Abstract

Objective: The sensitivity of diagnosing otosclerosis in high-resolution temporal bone CT (HRCT) examinations varies considerably in various studies. In this study, we investigated the effectiveness of temporal bone HRCT in patients with otosclerosis and the extent to which sharing the patient's clinical preliminary diagnosis with radiologists contributed to their ability to diagnose otosclerosis on temporal bone HRCT. Materials and Methods: The temporal bone HRCT reports and operative data of 36 patients who underwent stapedectomy for otosclerosis were retrospectively analyzed. After obtaining detailed clinical information about the patient, the temporal bone HRCT images were re-evaluated by the same two radiologists who had previously reported the images. Results: Otosclerosis was diagnosed with approximately 40.4 % in the analysis performed by general radiologists without information about preliminary diagnosis and 73.8 % in the second evaluation with clinical preliminary information. In 6 ears, otosclerosis could not be diagnosed by temporal bone HRCT despite the evaluation performed after the clinical preliminary diagnosis was known. Conclusion: The effectiveness of temporal bone HRCT examination in the diagnosis of otosclerosis varies. In patients with suspected otosclerosis, communicating the preliminary clinical diagnosis to radiologists significantly increases the diagnostic sensitivity of temporal bone HRCT.
