TİP I DENS İNVAGİNATUS VE TALON TÜBERKÜLÜNÜN BİRLİKTE GÖZLENDİĞİ MAKSİLLER LATERAL KESİCİ DİŞTE ENDODONTİK TEDAVİ: BİR OLGU NEDENİYLE

Abstract


Amaç: Dens invaginatus diş gelişimi sırasında dental papillanın katlanması sonucu oluşan dental malformasyondur ve dens in dente olarak da adlandırılmaktadır. Diğer bir dental malformasyon ise anterior dişlerin lingual yüzeylerinde az gelişmiş bir tüberkül olarak gözlenen talon tüberkülüdür. Bu iki dental anomali çok nadir olarak aynı dişte beraber görülebilmektedir. Bu vaka raporunda dens invaginatus ve talon tüberkülünün beraber görüldüğü maksiller sağ lateral kesici dişin teşhisi ve endodontik tedavisi bildirilmiştir.


Olgu Raporu: 37 yaşında bayan hasta Kocaeli Üniversitesi Diş Hekimliği Fakültesine maksiller lateral kesici dişinde orta şiddette ağrı şikayeti ile başvurmuştur. Başlangıç radyografisinde talon tüberkülü ve tip I dens invaginatusa eşlik eden periapikal lezyon gözlemlenmiştir. Kronik apikal periodontitis tanısıyla konvansiyonel endodontik kök kanal tedavisi uygulanmıştır.


Bulgular: 12 aylık klinik takip sonucunda dişin asemptomatik olduğu ve periapikal lezyonda iyileşme olduğu tespit edilmiştir.


Sonuçlar: Doğru teşhis ve tedavi yaklaşımı için tam bir klinik muayene ve radyolojik yorumlama çok önemlidir. Diş anomalileri ile birlikte görülen çürüğün erken tanısı pulpa iltihabı gibi birçok patolojinin önlenmesine olanak sağlar.

ENDODONTIC TREATMENT OF CONCURRENT OCCURANCE OF TYPE I DENS INVAGINATUS AND TALON CUSP IN MAXILLARY LATERAL INCISOR: A CASE REPORT

Abstract


Aim: Dens invaginatus, also known as dens in dente, is a tooth malformation caused by infolding of the dental papilla during tooth development. Another tooth malformation talon cusp is accessory tubercles located on the lingual faces of anterior teeth. Concurrent occurence of these anomalies in a single tooth is rare. This case report presents the diagnosis and nonsurgical endodontic management of dens invaginatus associated with talon cusp within the same tooth.


Case Report: A 37 years old female patient referred to the Faculty of Dentistry, Kocaeli University, with a history of moderate pain. Initial periapical radiograph revealed the presence of type I dens invaginatus associated with talon cusp in maxillary right lateral incisor tooth. There was a well defined periapical lesion related to tooth anomaly. A diagnosis of chronic apical periodontitis was made. Conventional endodontic treatment was performed.


Results: Follow-up examination 12 months later showed that the tooth was asymptomatic. The radiological findings presented resolution of the periapical radiolucent area.


Conclusions: Thorough clinical examination and radiographic interpretation are important to provide accurate diagnosis and treatment approach. Early diagnosis of caries which associated with dental abnormalities is so critical otherwise it might cause dental problems such as pulpal infection.
