Hidrosefalili Çocuğa Sahip Ebeveynlerin Yaşam Kalitesi ve Hemşirenin Rolü

Abstract

Hidrosefali,
beyin omurilik sıvısının (BOS) salgılanması ve emilimi arasındaki çeşitli
nedenlere bağlı dengenin bozulması sonucu ventriküler sistemin genişlemesi ve
beyin dokusuna baskı yapması ile seyreden ve çocuk ve aileyi yaşam boyu etkileyen
bir durumdur. Dünya genelinde 1000’de 3-4 olarak görülmektedir. Cerrahi tedavide
ventrikülo-peritoneal şant tercih edilir. Çocuklarda şant takılmasını izleyen
ilk yıl içinde şantın çalışmama oranı %40’tır. Buna sıklıkla, şant enfeksiyonu,
şantın tıkanması ve şantın yetersiz fonksiyon görmesi neden olmaktadır. Tedavi
sonrası ilk bir yılda bu tür komplikasyonlara bağlı çocukların hastaneye sık yatırılması,
çocuk ve ailenin yaşam kalitesini olumsuz etkilemektedir. Bu süreçte aileler, fiziksel,
duygusal, sosyo-ekonomik pek çok sorunla karşılaşmaktadır. Hemşirelere çocuk ve
ailelerin bu sorunlarla başa çıkmalarında önemli roller düşmektedir.  Yeni tanı alma durumunda, aile olayı anlamakta
zorlanır, konuyu öğrendikçe en sık beyin hasarı, zekâ geriliği veya diğer kalıcı
komplikasyonlardan korkar. Hemşireler, ailelerin sorunu anlamaları ve komplikasyonların
önlenmesi konusunda eğitim ve çocuk ve ailenin yaşam kalitesinin artırılmasında
gerekli sosyal desteği sağlamada en yakın sağlık çalışanıdır. Bu derleme makalede
hidrosefalinin nedenleri, tedavi yaklaşımları, komplikasyonlar, çocuk ve
ailenin yaşam kalitesi ve hemşirelik rolleri tartışılmıştır.
