SKOLYOZUN KONSERVATİF TEDAVİSİNDE KORSE YAKLAŞIMI

Abstract

Skolyoz omurganın frontal düzlemde 10°'nin üzerindeki lateral sapmasını, horizontal düzlemde aksiyal (vertebral) rotasyonu ve sagital düzlemdeki (kifoz ve lordoz) fizyolojik eğriliklerin deği-şimini içeren üç boyutlu yapısal bir deformitedir. Skolyoz genel olarak yapısal ve fonksiyonel olarak ikiye ayrılır. Yapısal skolyoz idiyopatik (nedeni belli olmayan) ve non-idiyopatik (altta yatan bir nedeni olan- konjenital, sendromik vb.) alt başlıkları altında incelenmektedir. İdiyopatik skolyoz omurganın yapısal deformitelerinin %70’ini, adölesan idiyopatik skolyoz (AİS) ise idiyopatik skolyozların %80'nini oluşturmaktadır. Görülme sıklığının fazla olması nedeni ile konservatif tedavi yaklaşımları AİS üzerinde yoğunlaşmaktadır. Konservatif tedavi yaklaşımları egzersiz, skolyoza özel egzersizler, manuel tedavi uygulamaları, skolyoza özel yoğun rehabilitasyon programı ve korse tedavisini içermektedir. Korse tedavisinin etkinliğini belirtilen birçok çalışma bulunmakta ve korse tedavisi konservatif tedavi yaklaşımları arasında en etkili yöntem olarak görülmektedir. Bu derleme makalesi ile günümüzde skolyozun konservatif tedavisinde korse yaklaşımının yerini ve önemini vurgulamak amaçlanmıştır.
