Bir Danışanın Rüyalarında Kendiliğin Yeniden Örgütlenmesi: Bütüncül Bir Klinik İnceleme

Abstract

Bu çalışma, uzun süreli psikoterapi sürecinde rüyalarını düzenli olarak kaydeden genç yetişkin bir kadın danışanın rüya anlatılarını bütüncül psikoterapi perspektifiyle incelemektedir. Rüyalar; Freud’un dürtü-çatışma kuramı, Klein’ın nesne ilişkileri yaklaşımı, Bion’un düşünce oluşumu ve container–contained modeli ile Kohut’un kendilik psikolojisi çerçevesinde ele alınmıştır. Çalışma, rüyaların danışanın kendilik örgütlenmesindeki dönüşümü, duygulanım düzenleme kapasitesini ve terapötik ilişkiyle kurulan bağın zaman içindeki evrimini nasıl yansıttığını ortaya koymayı hedeflemektedir. Araştırma, nitel, klinik ve boylamsal tek olgu tasarımına dayanmaktadır. Birincil veri, danışanın terapi süreci boyunca tuttuğu rüya günlüklerinden oluşmuştur. Rüya anlatıları içerik analizi yöntemiyle incelenmiş; semboller, duygusal tonlar, kişilerarası etkileşimler ve eylem örüntüleri tematik olarak kodlanmıştır. Analiz, hem kesitsel hem de boylamsal düzeyde yürütülmüş; rüya temalarının zaman içindeki sürekliliği ve dönüşümü değerlendirilmiştir. Analiz sonucunda rüya içerikleri dört ana tema altında toplanmıştır: (1) kendilik sınırları ve sınır ihlalleri, (2) otorite ve ilişkisel çatışmalar, (3) kaygı, engellenme ve yetersizlik algısı, (4) haz, dürtü ve otonomi. Boylamsal bulgular, rüya kendiliğinin terapi süreci boyunca pasif ve kırılgan bir yapıdan, sınır koyabilen ve aktif eylemlilik sergileyen bir yapıya doğru evrildiğini göstermektedir. Rüyalar, terapistle kurulan ilişkinin de örtük yargılanma kaygısından açık negatif aktarıma, ardından onarım ve işlevselleşmeye uzanan dönüşümünü yansıtmıştır. Tartışmada, bu dönüşümlerin Bion’un alfa işlevi ve metabolizasyon kavramsallaştırması ile Kohut’un kendilik nesnesi ihtiyaçları ve dönüştürerek içselleştirme kavramları üzerinden gerekçelendirilmiştir. Çalışma, rüyaların bütüncül psikoterapide hem klinik değerlendirme hem de değişim izlemi için önemli klinik bir malzeme sunmaktadır.

The Reorganization of the Self in a Patient’s Dreams: An Integrative Clinical Case Study

Abstract

This study explores the role of dreams in the reorganization of the self within a long-term psychotherapy process from an integrative psychotherapy perspective. The dream narratives of a young adult female patient who consistently recorded her dreams throughout therapy were examined using a qualitative, longitudinal single-case design. Dreams were conceptualized through multiple theoretical frameworks, including Freud’s drive–conflict theory, Klein’s object relations approach, Bion’s theory of thinking and container–contained model, and Kohut’s self psychology. The primary data consisted of dream diaries collected during the therapeutic process. Dream narratives were analyzed using qualitative content analysis, with thematic coding of symbols, affective tones, interpersonal dynamics, and action patterns. The analysis was conducted both cross-sectionally and longitudinally to capture continuity and transformation over time. Four main thematic domains emerged: (1) self-boundaries and boundary violations, (2) authority and relational conflicts, (3) anxiety, inhibition, and perceived inadequacy, and (4) pleasure, drive, and autonomy. Longitudinal findings indicated a transformation of the dream self from a passive and fragile organization toward a more active, agentic structure capable of asserting boundaries. Dreams also reflected changes in the therapeutic relationship, moving from implicit fears of judgment to explicit negative transference, followed by processes of repair and integration. These findings are discussed in relation to Bion’s alpha function and Kohut’s concepts of selfobject needs and transmuting internalization, highlighting the clinical value of dreams in integrative psychotherapy.
