65 YAŞ VE ÜZERİ İNMELİ BİREYLERİN BİLİŞSEL FONKSİYONLARI VE AKILCI İLAÇ KULLANIM DURUMLARI

Öz

ÖZ
Giriş ve Amaç: Bu çalışma, 65 yaş ve üzeri inmeli bireylerin bilişsel fonksiyonlarını ve akılcı ilaç kullanım durumlarını belirlemek amacıyla yapılmıştır.
Gereç ve Yöntem: Kesitsel türdeki bu çalışmanın örneklemini, 04 Ağustos 2021- 14 Şubat 2022 tarihleri arasında bir üniversitenin Eğitim ve Araştırma Hastanesi Nöroloji servisinde inme tanısı ile yatan 65 yaş üzeri, 82 birey oluşturmuştur. Araştırmada veriler, Hasta Tanıtım Formu, Akılcı İlaç Kullanımı Yatan Hasta Anketi ve Standardize Mini Mental Test kullanılarak toplanmıştır. Araştırmadan elde edilen veriler SPSS 25.0 paket programında analiz edilmiştir.
Bulgular: İnmeli bireylerin %81,7’sinin iskemik inmesinin, %70,7’sinin 75 yaş altı, %57,3’ünün erkek, %46,3’ünün ise ilköğretim mezunu olduğu belirlenmiştir. Bireylerin %72,0’sinin hipertansiyon tanısı, %98,8’inin sürekli kullandığı ilacının olduğu, 40,2’sinin 5 ve daha fazla ilaç kullandığı ve %53,7’sinin 3-6 yıldır ilaç kullandığı, %80,5’inin kullandığı ilaçların etkisini bildiği, %47,6’sının ilaçların son kullanma tarihine dikkat etmediği, %58,5’inin kendi kendine ilaç kullandığı belirlenmiştir. İnmeli bireylerin %57,3’ünün gerektiğinde tedavi sonrası arta kalan ilaçları kullanmayı tercih ettiği, %69,5’inin evinde hiç kullanılmamış veya yarım kalmış 1-5 kutu ilacın olduğu ve %48,8’inin her yıl kutusu bile açılmadan 1-3 kutu ilacı son kullanım tarihi geçtiği için attığı saptanmıştır. Eğitimsizler için mini mental test toplam puan ortalaması 24,80±1,43, eğitimliler için ise 25,67±2,01’dir. Cinsiyet (p=0.001), genel olarak sağlığını değerlendirme (p=0.003), sağlık kuruluşlarına başvuru sıklığı (p=0.024) ve ilaçları içerken ayırt etmede kullanılan yöntemler (p=0.003) ile bilişsel performans arasında anlamlı fark saptanmıştır.
Sonuç: Bu çalışmada 65 yaş ve üzeri inmeli bireylerin akılcı ilaç kullanım ilkelerine uymayan davranışlar gösterdiği belirlenmiştir. Sonuçlar doğrultusunda yaşlı bireylerde akılcı ilaç kullanımını etkileyen faktörlerin araştırılıp oluşabilecek sağlık sorunlarının önüne geçilmesi önerilebilir.

Cognıtıve Functıons And Ratıonal Drug Use In Stroke Patıents Aged 65 Years And Older

Öz

ABSTRACT
Objective: The aim of this study was to determine the cognitive functions and rational use of medication in stroke patients aged 65 years and older.
Material and Methods: The sample of this cross-sectional study consisted of 82 individuals over the age of 65 who were hospitalized with a diagnosis of stroke in the Neurology Department of a university Training and Research Hospital between 04 August 2021 and 14 February 2022. Data in the study were collected using the Patient Introduction Form, Rational Drug Use Inpatient Survey and Standardized Mini Mental Test. According to the Post Hoc Power analysis result, the power of the study was determined to be 80%. Statistical significance level was accepted as p<0.05.
Results: It was determined that 81.7% of stroke individuals had ischemic stroke, 70.7% were under 75 years of age, 72.0% had a diagnosis of hypertension, 98.8% had continuous medication, 40.2% used 5 or more medications, 53.7% had been taking medication for 3-6 years, 80.5% knew the effects of the medications, 47.6% did not pay attention to the expiry date of the medications, and 58.5% self-medicated. It was found that 57.3% of the individuals with stroke could use their leftover medicines after treatment, when necessary, 69.5% had 1-5 boxes of medicines at home that were never used or left unfinished, and 48.8% discarded 1-3 boxes of medicines every year without even opening the box because the expiry date had passed. The mean mini mental test total score for the uneducated is 24.80±1.43, and for the educated it is 25.67±2.01. A significant difference was found between gender (p=0.001), general health assessment (p=0.003), frequency of admission to health institutions (p=0.024), and methods used to distinguish (p=0.003) between taking medications and cognitive performance.
Conclusion: In this study, it was determined that individuals with stroke aged 65 and over showed behaviors that did not comply with the principles of rational drug use. In line with the results, it may be recommended to investigate the factors affecting rational drug use in elderly individuals and prevent possible health problems.
