Challenges of Performance Management for Health-care in Public Sector: The Case of Turkey

Abstract

In Turkey, neoliberal health reforms gained acceleration with Heath Transformation Programme which has been practiced since 2003. In this context, primary health-care has been transformed into “family medicine model”, secondary and tertiary health-care have been converted to “public hospital unions”, and financing and delivery of health-care have been separated from each other, General Health Insurance has been established, and performance management system which establishes relationship between produced health-care and wages has been put into practice as payment system. However, the system has both some significant challenges in practice and unfavourable impacts on health-care system. In this sense, the study aims to discuss the impacts of performance management system for health-care in public sector on team service dimension of health-care, quality of health-care, health expenditures, unfairness in performance payments, redundant medical interventions and overuse of medical equipments and drugs. The study analyses secondary data from the public institutions by using descriptive analysis method. Fundamentally, in the research, it is argued that performance management for health-care in public sector has some challenges and unfavourableness in practice.
