Migration of sequestrated part to posterior epidural region in lumbar disc herniation

Abstract

Lumbar disc herniation
occurs with the emergence of the nucleus pulposus from weakened annulus
fibrosus fibers. Lumbar disc herniation can be seen as protruding, extruded and
sequestered type. Sequestered parts often replace to superior, inferior and
laterally. Migration of disc fragment to the posterior epidural space is
extremely rare. Here, two cases with posterior epidural migration were
reported. One patient had radicular symptom and other one had cauda equina
syndrome. In the lumbar magnetic resonance imaging of the patients, there was a
mass at the posterior epidural space, compressing the dural sac. The patients
underwent emergency operation and the lesions at the epidural space were proved
to be sequestered disc fragments which migrated to posterior epidural space.
Clinical symptoms of the patients were improved after surgery and the patients
were discharged with full recovery.

Lomber disk hernisinde sekestre parçanın posterior epidural mesafeye migrasyonu

Abstract

Lomber disk hernisi
zayıflamış annulus fibrozus liflerinden nucleus pulposusun çıkması ile
oluşmaktadır. Lomber disk hernisi protrude, ekstrude ve sekestre şekilde
görülebilir. Sekestre parçalar sıklıkla superior, inferior ve laterale doğru
yer değiştirebilirler. Serbest parçanın posterior epidural mesafeye göçü ise
son derece nadir olarak rapor edilmiştir. Burada posterior epidural mesafeye
göç etmiş olan iki disk olgusu sunulmuştur. Bir olguda radiküler semptom, bir
olguda kauda equina sendromu mevcuttu. Hastaların lomber manyetik rezonans
görüntülemesinde posterior epidural mesafeden dural sakı basıya uğratan kitle
mevcuttu. Hastalar tarafımızca acil operasyona alınmış ve operasyon sırasında
epidural alandaki kitlenin posterior epidural alana göç etmiş sekestre disk
parçası olduğu görülmüştür. Operasyon sonrası klinik semptomlarında düzelme
olan hastalar şifa ile taburcu edilmiştir.
