First Episode Mania Probably Associated with Discontinuation of Paroxetine Treatment

Abstract

Antidepressant-induced mania has been reported with all major antidepressant classes in a subgroup of about 20-40% of bipolar patients. Mania/hypomania due to antidepressant withdrawal is rarely reported in the literature as case report. Mania/hypomania due to antidepressant withdrawal is reported to be associated with tricyclic antidepressants as well as selective serotonin reuptake inhibitors. This article offered mania occurring after discontinuation of treatment with paroxetine male patient. Mania associated with antidepressant discontinuation of the differential diagnosis, possible mechanisms, and the treatment has been discussed in the literature. Clinicians should be cautious mania/hypomania associated with antidepressant withdrawal. To do follow-up studies and case reports will help to clarify this table.

Olası Paroksetin Tedavisinin Kesilmesi İle İlişkili İlk Atak Mani

Abstract

Antidepresan ilaç kullanan duygudurum bozukluğu olan hastalarda hipomanik/manik kayma riskinin %20-40 arasında olduğu bildirilmektedir. Antidepresan ilaçların azaltılması yada kesilmesi ile ortaya çıkan hipomani/mani ise olgu sunumlarıyla sınırlıdır. Daha çok trisiklik antidepresanların kesilmesi ile ortaya çıkmakla beraber seçici serotonin geri alım inhibitörlerinin kesilmesi ile de bildirilmiştir. Bu yazıda paroksetin tedavisinin kesilmesini takiben mani tablosu ortaya çıkan erkek hasta sunulmuş, antidepresan kesilmesi ile ortaya çıkan maninin ayırıcı tanısı, olası mekanizmaları, tedavisi literatür ışığında tartışılmıştır. Klinisyenler antidepresan kesimi ile ilişkili mani/hipomani açısından dikkatli olmalıdır. Bu alanda yapılacak izlem çalışmaları ve olgu bildirimleri bu tablonun aydınlatılmasına yardımcı olacaktır.
