FARKLI SAGITTAL ISKELETSEL PATERNE SAHIP YETIŞKIN TÜRK HASTALARDA HYOID KEMIK KONUMUNUN DEĞERLENDIRILMESI

Abstract

Amaç: Değişik anteroposterior paterne sahip, yetişkin türk bireylerde hyoid kemik pozisyonunu değerlendirmek amaçlanmıştır.
Yöntemler: Retrospektif çalışmamızda kullanılmak üzere 61 hastaya (yaş ortalaması 25,62 ± 5,87 yıl) ait sefalometrik film arşivden seçilmiş; ANB açısı değerine göre 19 adet Sınıf I (9 erkek, 10 kadın), 26 adet Sınıf II (4 erkek, 22 kadın) ve 16 adet Sınıf III (8 erkek, 8 kadın) olmak üzere alt gruplara ayrılmışlardır. Hyoid kemiğin vertikal ve sagittal konumunu belirlemek üzere, röntgenler üzerinde 6 açısal, 7 çizgisel ölçüm yapılmış ve istatistiksel olarak fark olup olmadığı değerlendirilmiştir.
Bulgular: Çalışmamızın bulguları, kontrol grubu (Sınıf I) ve çalışma grupları (Sınıf II ve Sınıf III) arasında hem sagital hem de dikey düzlemlerde önemli farklılıklar olduğunu ortaya koymuştur. Hyoid kemiğin anterior kraniyal tabana göre sagittal düzlem açısal ölçümü (NSH) ile birlikte, H-NPer parametresinde kontrol ve çalışma grupları arasında istatistiksel olarak anlamlı farklılıklar bulunmuştur. Buna karşın, hyoid kemik vertikal pozisyonu açısından, açısal ve doğrusal ölçümler (MP-H, H-SN, H-FH, H-MP, H-PP) Sınıf I, Sınıf II ve Sınıf III gruplarında, istatistiksel olarak anlamlı bir fark göstermemiştir. Cinsiyetler arası karşılaştırma yapıldığında ise; her 3 grupta da erkeklerin hyoid kemikleri, kadınlara göre daha aşağıda konumlanmış bulunmuştur.
Sonuç: Sagital düzlemde: Hyoid kemiğin, Sınıf II maloklüzyon vakalarında posteriorda, Sınıf III vakalarında ise anteriorda konumlandığı tespit edilmiştir. Vertikal düzlemde ise: Hyoid kemik, Sınıf III vakalarda Mandibuler düzleme daha yakın, Sınıf II vakalarda daha aşağıda ve uzakta konumlandığı belirlenmiştir. Ayrıca, her 3 grupta da erkeklerin hyoid kemikleri, kadınlara göre daha aşağıda konumlanmış bulunmuştur.

Abstract
Objective: Our aim was to evaluate the hyoid bone position in adult Turkish individuals with different anteroposterior patterns.
Methods: For this retrospective study, 61 cephalometric films of 61 patients with a mean age of 25.62 ± 5.87 years; were selected from the archive; according to the ANB angle value, they were divided into subgroups respectively as 19 Class I (9 male, 10 female), 26 Class II (4 male, 22 female) and 16 Class III (8 male, 8 female). In order to determine the vertical and sagittal position of the hyoid bone, 6 angular and 7 linear measurements were made on the x-rays. The data was statistically evaluated.
Results: The findings of our study revealed that there were significant differences between the control group (Class I) and study groups (Class II and Class III) in both the sagittal and vertical planes. Along with sagittal plane angular measurement of the hyoid bone with respect to the anterior cranial base (NSH), statistically significant differences, between the control and study groups, were found also in the H-NPer parameter. On the other hand, angular and linear measurements (MP-H, H-SN, H-FH, H-MP, H-PP) did not show a statistically significant difference between groups in terms of vertical hyoid bone position. When comparing the genders; we found that, in all 3 groups, the hyoid bones of males were located lower than females.
Conclusion: In the sagittal plane: It has been determined that the hyoid bone is located posteriorly in Class II malocclusion cases and anteriorly in Class III cases. In the vertical plane: the hyoid bone is located closer to the Mandibular plane in Class III cases, and lower and farther away in Class II cases. In addition, hyoid bones of males were located lower than females in all 3 groups.
