Abdominal Aort Anevrizmasında Endovasküler Tedavi Sonrası Anevrizma Kesesi, Trombüs Çapı ve Yükünün Değerlendirilmesi ve Risk Faktörleri ile İlişkisinin İrdelenmesi

Abstract

Am: Abdominal aort anevrizmalarının endovasküler
onarım sonrasında anevrizma morfolojisinin yeniden modellenmesinde, anevrizma
çapı, trombüs çapı ve yükündeki erken dönem değişikliklerin değerlendirilmesi
ve hasta grubunun risk faktörleri ile bağlantılı olarak prognozun ve tedavinin
belirlenmesi amaçlandı.

Ger ve yöntem: Aort anevrizması ön tanısıyla bilgisayarlı tomografi anjiyografi (BT-A)
tetkiki ile değerlendirilen ve endovasküler tedavi için uygun görülen 150 hasta
çalışma kapsamında incelendi. İşlem öncesi ve 1., 3., 6. ve 12. ay sonrasında
radyolojik izlemleri BT-A ile yapılan 37 hasta çalışmaya dahil edildi. Kalan hastalar,
BT-A tetkiklerinin dış merkezli olmasından dolayı değerlendirmeye alınmadılar.
Çalışmaya dahil edilen 52 ile 93 yaşları arasındaki (ortalama 69,08) 32 erkek,
5 kadın hastanın preoperatif ve postoperatif BT-A görüntüleri arşivden alınarak
retrospektif değerlendirildi. Aksial, koronal ve sagittal görüntülerde
anevrizma kese çapı, trombüs çapı ve trombüs yük ölçümleri yapıldı. Hastaların
demografik özellikleri arşiv kayıtlarından ve merkez labaratuvar verilerinden
elde edildi.

Bulgular: On iki aylık dönemde endovasküler tedavi
sonrasında kese çapında 7,4 mm küçülme olduğu gözlendi (p<0,001). Postoperatif 1. ayda ortalama trombüs çapında artış
izlenirken, zamanla azalma olduğu görüldü (p<0,001).
Trombüs yükünde de 1. ay sonrasında artış izlenirken, ilerleyen takiplerde
azalma olduğu saptandı (p<0,001).
Söz konusu değişimlerin yandaş hastalıklardan ve sigara kullanımından bağımsız
olduğu gözlendi. Sadece ikili antikoagülan kullanan grupta tip II endoleak
sıklığı daha fazla bulundu.







Sonuç: Preoperatif-postoperatif anevrizma morfolojisinin karşılaştırılması,
tedavinin etkinliğinin değerlendirilmesine ve komplikasyonların öngörülmesine
olanak sağlayabilir. Değerlendiğimiz üç kriterde de endovasküler aortik anevrizma
onarımı (EVAR) sonrasında anlamlı değişiklikler gösterilmiştir. Veriler
ışığında 
anevrizma kese çapı, trombüs çapı ve yükündeki değişiklerin ve endoleak
oluşumunun prognozu etkileyeceğini ve tekli antiplatelet tedavisinin daha uygun
olduğunu düşünüyoruz.
