A Case of Neonatal Sepsis due to Listeria monocytogenes Serotype 4b: Case report and literature review

Abstract

Listeria monocytogenes has been often leads to infection in pregnant women, newborns and patients with impaired immune response. Serotypes 1/2a, 1/2b and 4b are responsible on majority of human infections and outbreaks of food. The girl baby was born from twenty years old, primiparous mothers at term with normal birth weight in our hospital. She was brought with to the pediatric emergency department with fever, groaning, sucking, bruising and complaints on the second day of the birth. Ampicillin and cefotaxime was started to patient with diagnosis of early-onset neonatal sepsis. First blood culture sample of the patient were positive on the second day of incubation. Then, Gram-positive bacilli were seen in gram stain. It was made identification of bacteria in two different automated systems (Phoenix and VITEK).We believed that this case is important in the way of case of neonatal sepsis with Listeria monocytogenes serotype 4b that firstly ever reported in Turkey.

Listeria monocytogenes serotip 4b\'ye bağlı neonatal sepsis: Bir olgu ve literatür derlemesi

Abstract

Listeria monocytogenes çoğunlukla yenidoğanlar ve hamileler gibi immün yanıtı bozuk olan hastalarda enfeksiyona yol açmaktadır. İnsan enfeksiyonlarında ve gıda salgınlarının çoğunluğunda sorumlu serotipler 1/2a, 1/2b ve 4b\'dir. Yirmi yaşındaki annenin ilk gebeliğinden, miadında normal doğum ile hastanemizde doğan kız bebek, doğumunun ikinci gününde ateş, inleme, morarma ve emmeme şikayetleri ile pediatri servisine başvurdu. Erken başlangıçlı neonatal sepsis tanısı ile ampisilin ve sefotaksim başlandı. İlk gün alınan kan örneğinin, inkübasyonun ikinci gününde pozitif sinyal vermesi üzerine, 24 saatlik inkübasyonda, yarı saydam, dar bir beta hemoliz zonu yapmış koloniler gözlendi. İki farklı otomatize sistemde (Phoenix ve VITEK) tanımlama yapıldı. Doğrulama ve serotiplendirme için Halk Sağlığı Kurumu Mikrobiyoloji Referans Laboratuarlarına gönderildi. Literatürde Listeria monocytogenes serotip 4b\'ye bağlı neonatal sepsis olgu sayısı sınırlı sayıda bulunmaktadır. Bu olgunun; neonatal sepsisin etkeni olarak nadir görülen ve şimdiye kadar Türkiye'de bildirilen ilk Listeria monocytogenes serotip 4b neonatal sepsis olgusu olması açısından önemli olduğunu düşünmekteyiz.
