Difficulties in diagnosis of hemophagocytic lymphohistiocytosis: Case report

Abstract

In 1991, diagnostic guidelines for hemophagocytic lymphohistiocytosis were presented by the Histiocyte Society; however, a number of patients may develop one or more of the diagnostic criteria late during the course of the disease. With these concerns in mind, the diagnostic guidelines have been revised at 2004. Delay in diagnosis may result in high morbidity and mortality. If we don\'t verify the patient has a genetically disease or a familial form of hemophagocytic lymphohistiocytosis, and if the disease is severe, persistent, or recurrent, we have to start specific therapy for hemophagocytic lymphohistiocytosis. Here, we report the difficulties in diagnosis and treatment in a case with an atypical and insidious course in whom all criteria are not fulfilled at the beginning, and although the corticosteroid usage for suspicious underlying disease made the diagnosis conflict, and delayed criteria for hemophagocytic lymphohistiocytosis. Hemofagositik lenfohistiyositozis tanısında zorluklar: Olgu sunumu

Abstract

Histiosit Grubu tarafından hemofagositik lenfohistiyositozis tanı kriterleri 1991 yılında yayınlanmıs, ancak bazı hastalarda tanı için gerekli kriterlerin bir veya birkaçının hastalığın geç dönemlerine kadar görülemeyebildiği belirlenerek hemofagositik lenfohistiyositozis tanı ve tedavi kılavuzu 2004 yılında yenilenmistir. Tanıda ve tedavi baslanmasındaki gecikme morbidite ve mortalite ile sonuçlanabilmektedir. Hemofagositik lenfohistiyositozisin ailevi mi, genetik zeminde mi gelismis olduğunun belirlenemediği sekonder olması muhtemel durumlarda; hastanın durumu ciddi, persistan ve rekürren ise hemofagositik lenfohistiyositozise yönelik tedavi baslama endikasyonu vardır. Burada baslangıçta tanı için gerekli kriterlerin hepsini tasımayan bir hastamızda, hemofagositik lenfohistiyositozisin atipik seyrini ve altta yatan hastalığın kesinlestirilememesine rağmen hastanın genel durumunun bozulması ile baslanan steroid tedavisinin tanı kriterlerinin bir araya gelmesini geciktirdiği bir olgumuzun tanısında ve tedavisinde yasanan güçlükleri tartısmayı amaçladık.
