Comparison of QT dispersion between subclinical hypothyroid and euthyroid patients

Abstract

Objectives: The aim of this study was to investigate the relationship between subclinical hypothyroid and QTc dis­persion indicating local heterogeneity in repolarization of myocardium, which is well known as independent cardiac risk factor for sudden death and ventricular arrhythmia. Materials and Methods: We compared QTc dispersion of subclinical hypothyroid patients, after treatment and healthy control group. We included a total of 50 patients with 41 women and 9 men in the study group. Electrocar­diography with 12 derivations, thyroid hormones, serum electrolytes and basic biochemical parameters were mea­sured. The control group consisted of 25 healthy individu­als. QT distances were calculated by using Bazet formula. The difference between the longest QTc and the shortest QTc distance was accepted as QTc dispersion (QTcd). Results: Comparison of subclinical hypothyroid patients, their euthyroidic period after treatment and healthy con­trol group, gave no significant differences in age, body weight, body mass index and free thyroxin values. How­ever, significant difference was found in durations of QTd and QTcd between the subclinical hypothyroid, the control and the euthyroidic groups (p0.05). Conclusion: Our results suggested that subclinical hy­pothyroid patients had longer QTc dispersion compared to euthyroidic period and healthy subjects. However there was no QTcd difference between the euthyroidic period and healthy control group.

Subklinik hipotiroidili hastalardaki QT dispersiyonunun ötiroid hastalarla karşılaştırılması

Abstract

Amaç: Subklinik Hipotiroidi en sık görülen tiroid fonksiyon bozukluklarındandır. Hastaların birçoğu asemptomatiktir ve bu nedenle çoğu zaman tanı konamamaktadır. Bağım­sız bir kardiak risk faktörü olan ventriküler aritmi ve ani ölüm riskini artırdığı bilinen miyokard repolarizasyonun­daki bölgesel heterojeniteyi gösteren QT mesafesi (QTc) dispersiyonunun subklinik hipotroidi ile ilişkisini saptamak amacıyla bu çalışmayı planladık. Gereç ve Yöntem: Hastanemiz endokrinoloji polikliniğin­de izlenen subklinik hipotiroidili hastaları, tedavi sonrası ötiroidik olduğu dönemi ve sağlıklı kontrol grubunu karşı­laştırdık. Herhangi bir kalp hastalığı, elektrolit dengesiz­liği ve QT mesafesini etkileyen bir ilaç kullanan hastalar çalışma dışı bırakıldı. Çalışmaya 41\'i kadın, 9\'u erkek olmak üzere toplam 50 hasta alındı. Hastalarda elektro­kardiyografi, tiroid hormonları, elektrolitler ve biyokimya­sal parametreler değerlendirildi. Toplam 25 sağlıklı birey kontrol grubu olarak alındı. Bazzet formülü kullanılarak düzeltilmiş QT mesafeleri hesaplandı. En uzun QTc ile en kısa QTc mesafesini farkı QTc dispersiyonu (QTcd) olarak hesaplandı. Bulgular: Subklinik Hipotiroidili hastalarla bu hastaların ötiroid olduğu dönemi ve sağlıklı kontrol grupları arasında yaş, vücut ağırlığı, vücut kitle indeksi ve serbest tiroksin düzeyleri bakımından anlamlı fark saptanmazken; subk­linik hipotiroili grup ile kontrol ve ötiroidik grup arasında QTd ve QTcd süreleri arasında istatistik açıdan ileri dü­zeyde anlamlı fark saptandı (p0.05). Sonuç: Subklinik hipotiroidili hastaların ötiroid oldukla­rı döneme ve kontrol grubuna kıyasla daha uzun QTcd sahip olduğu görüldü. Subklinik hipotirodili hastalardaki QTcd uzaması hastalar tedaviyle ötiroid hale geldiklerinde sağlıklı kontrol grubuyla farksız hale geldi.
