Unexpected difficult intubation due to subglottic ring

Abstract

Airway damages encountered during endotracheal intu­bation or tracheostomy may cause some complications such as severe dyspnea. Upper airway diagnostic endos­copy was planned to find the etiology of effort dyspnea in a 5 years old girl who had endotracheal intubation been performed during newborn period. Her ASA score was 1, and Mallampati score for preoperative airway evaluation was 1. Physical examination revealed neither dyspnea nor stridor while the patient was not exerting effort. Her chest radiograms were normal. She had no history of pre­vious surgical or anesthetical intervention. In this report we presented a difficult intubation during the endoscopic examination of upper airway in a patient who had dysp­nea while exerting effort.

Abstract

Endotrakeal entübasyon veya trakeostomi esnasında olu­şan hava yolu hasarı ciddi solunum sıkıntısı gibi kompli­kasyonlara neden olabilir. Yeni doğan döneminde yeni do­ğan yoğun bakım ünitesinde birkaç entübasyon yapılma öyküsü bulunan ve efor dispnesi olan 5 yaşında bir kız ço­cuğa, dispne etyolojisini bulmak için tanısal üst solunum yolları endoskopisi planlandı. ASA skoru I olan çocuğun operasyon öncesi hava yolu güvenliği açısından yapılan muayenesinde, Mallampati skoru I olarak değerlendirildi. Fizik muayenesinde efor olmaksızın ne solunum zorluğu ne de stridor izlendi. Akciğer filmleri normaldi. Olgu daha önce herhangi bir ameliyat ve anestezi uygulaması ge­çirmemişti. Bu olguda eforla solunum sıkıntısına giren bir hastanın tanısal amaçlı üst solunum yolu endoskopik mu­ayenesinde karşılaşılan zor entübasyon sunulmuştur.
