Chronic pain, psychological factors and depression

Abstract

Chronic pain is a common health problem in clinical prac­tice. Besides the increase in health-care expenditure, it also causes decline in patient\'s functioning and working capability. In addition, chronic pain and psychiatric symp­tom comorbidity is very frequent. Recently there are many studies focusing on depression and pain. It might be just a symptom of depression, or might affect psychological status as a consequence of physical disorder. As a result, multidisciplinary approach is necessary for a successful treatment. In this review generally chronic pain, psychological fac­tors and especially depressive disorders are summarized; and the multidisciplinary approaches to the diagnosis, as­sessment and treatment of the patient with chronic pain are overviewed.

Kronik ağrı, psikolojik etmenler ve depresyon

Abstract

Kronik ağrı klinik pratikte tüm klinisyenlerin sıkça karşı­laştığı önemli bir sağlık sorunudur. Sağlık harcamalarında ciddi artışın yanı sıra hastanın işlevselliğinin bozulması­na, işgücünün düşmesine yol açmaktadır. Kronik ağrı ve psikiyatrik belirti birlikteliği oldukça fazladır. Son zaman­larda depresyon ve ağrı üzerine yapılan çalışmalar dikkat çekicidir. Kronik ağrı, kimi zaman depresif bozukluğun bir belirtisi iken, kimi zaman da fiziksel bir bozukluk olarak kişinin ruhsal dünyasında bozulmalara yol açmaktadır. Bu nedenle kronik ağrılı hastaya disiplinler arası yaklaşım başarılı tedavinin önkoşulu olmuştur. Bu gözden geçirme yazısında genel olarak kronik ağrı, eşlik eden psikolojik etmenler ve özellikle depresif bo­zukluklar incelenmiş; kronik ağrılı hastanın tanı, değer­lendirme ve tedavisi ile ilgili multidisipliner yaklaşımlara değinilmiştir.
